2781384 Primary Facility 2018-09-28 Preoperation Inspection No Violations Noted The DEP conducted a PRE-SPUD inspection of the JKLM Judson Hollow 302 well pad located in Pike Twp, Potter Co. on 9/28/2018 @ 1330 hours. The weather was 63 degrees, winds NE 9mph,humidity 78%. No personnel were onsite during this inspection.

The site consists of an access road, multi-well pad and associated E&S controls. At the time of this inspection the well pad slopes and topsoil stockpile appeared stabilized by heavy mulch and ~10% vegetative cover of Annual Rye grass. No water was observed in ST-2 off the northern corner of the pad. A small amount of water was observed in ST-1 off the southern side of the pad.

Operator is reminded, a 6" thick layer of compost shall added to the upslope side of any wood chip filter berm located in an HQ watershed. No compost was observed on the chip filter berms during today's inspection.

This inspection meets the requirements of This inspection meets the requirements of 58 Pa. C.S. 3258 (a.1) regarding a "pre-operational inspection".

2799235 Primary Facility 2018-11-06 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was conductor only. Pipe is 20" in diameter ad set at a depth of 120'. Conductor pipe was set on 10-10-18 by Butches Rat Hole Service.
2822422 Primary Facility 2019-01-02 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the JKLM Judson Hollow 302 well pad located in Pike Twp, Potter Co. on 1/2/2018 @1430 hours. The weather was 32 degrees, overcast, winds S @3mph, 78% humidity. Ron Burdick, JKLM Company Man and Jared Jacobini, DEP WQS was present onsite for the inspection. The Falcon rig is currently onsite drilling the vertical portion of 1 marcellus and 3 utica formation wells. At the time of our inspection the rig was at a depth of 300' on the 4HM well.

Puddles were observed in the NE corner of the pad with a field measured conductivity of 2.5 mS/cm. The conductivity decreased as moving away from the area. Fluid in containment had a field measured conductivity of 500uS/cm. The source of the high conductivity fluid could not be located. The Operator representative onsite stated recent truck deliveries were made in the snow and coudl have tracked ice melt onto the site resulting in deposits. The Operator stated he would have a vac truck remove the liquid immediately. No violations

2823036 Primary Facility 2019-01-07 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was vertical drilled only. The conductor casing that is 20" x 120' was set on 10-10-18 thru 10-10-18. The hole for the surface casing that is 13 3/8" was being bored out at the time of this inspection and was at a depth of 418'. No surface casing pipe had been set in the hole at the time of this inspection. No violations were noted at this inspection.
2837559 Primary Facility 2019-02-06 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the JKLM Judson Hollow 302 well pad located in Hector Twp, Potter Co. on 2/6/2019 @1145 hours. The weather was 34 degrees, rain, winds N @10 mph. Ron Burdick, JKLM was present onsite for the inspection.

The top-hole Falcon rig was in the process of preparing to skid over to begin drilling the 3HU well. This is the 3rd of 4 wells planned to be drilled on the pad. Mr. Burdick anticipated being onsite for another 3 weeks.

No violations are noted at this time. I departed the site at 1330.
2838625 Primary Facility 2019-02-05 Drilling/Alteration No Violations Noted At the time of this inspection this wells top hole drilling was complete. The 1st intermediate (10 3/4"casing) was started on 1-25-19 and finished on 1-26-19. The 2nd intermediate (7 5/8"casing) was started on 2-5-19 and completed on 2-5-19. The Falcon Rig 38 and crew did this drilling. The H&P 500 Flex Rig and crew will set up and drill a bore hole for the 5" production casing and then advance 5" casing into the well at a future date. No violations were noted at this inspection.
