2740268 Primary Facility 2018-06-12 Preoperation Inspection No Violations Noted The DEP conducted a PREOP inspection of the JKLM Sweden valley 104 wells site located in Summit Township Potter County on June 12, 2018 at 1100 hrs. JKLM representative Jeff Long was present for the inspection. The weather was 68° sunny, 53% humidity, winds South at 13 mph.

No issues were observed. This inspection meets the requirements of 58 Pa. C.S. 3258 (a.1) regarding a "pre-operational inspection".

I departed the site at 1245 hours.

2745537 Primary Facility 2018-06-20 Routine/Complete Inspection No Violations Noted At the time of this inspection this wells conductor pipe 20" x 120' had not been set in a 120' bore hole .
2750628 Primary Facility 2018-07-10 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was a conductor only. Well is a 20" diameter by 120' in depth set conductor. This hole was drilled and the conductor was set on 6-25-18. The conductor was cemented on 6-26-18.
2761826 Primary Facility 2018-08-08 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was a conductor only. The conductor is 20" in diameter at 120' depth. The rigging up(prepping for drilling) was on going at the time of this inspection. Chris Wiley( drilling consultant) was present at this inspection. I was informed that the wells will be Batch Drilled. Batch drilling is the drilling and setting of the surface casing for each well, then the 1st intermediate for each well then the 2nd intermediate for each well and then the production casing for each well. I was informed that the anticipated spud in of the first well (5HU) was Sunday or Monday. No violations were noted at this inspection.
2763375 Primary Facility 2018-08-14 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was conductor only. At the time of this inspection the Falcon 38 Super Single Rig and crew were finishing rigging up to start drilling. The plan is to set surface casing on all five(5) of the wells then go back to each one to set the 1st and 2nd intermediate after the surface casing on all the wells is set. No violations were noted a the time f this inspection.
2767198 Primary Facility 2018-08-23 Routine/Complete Inspection No Violations Noted The DEP conducted a ROUTINE inspection of the JKLM Sweden Valley 104 well pad on 8/23/2018 @1230 hours. The weather was 70 degrees, sunny, 58% humidity windys WNW 12 mph. The access road, well pad and containment were inspected.

The Falcon Drilling 38 super single rig is onsite batch drilling, the surface casing, 1st, and 2nd intermediate string casings, on the 1HU-5HU wells. The access road, well pad and containment were inspected. The pad-wide containment was holding fluid in some areas with a field measured conductivity of <200uS/cm. The corners of the containment appeared folded down, with the potential to be overtopped. Onsite JKLM personnel stated they were monitoring the corners and working on a solution. Puddles on the pad surface had a field measured conductivity of <500uS/cm.

No violations are noted at this time. I departed the site at 1415 hours.

2767542 Primary Facility 2018-08-27 Routine/Complete Inspection No Violations Noted At the time of this inspection this well has had the conductor set and the surface casing was set on 8-19-18 to 8-21-18. Surface casing has been cemented with cement return to surface successfully. No violations noted at this inspection.
2797856 Primary Facility 2018-11-02 Routine/Complete Inspection No Violations Noted At the time of this inspection this well has the conductor (20"),the surface casing (13 3/8"), the first intermediate (10 3/4") and the second intermediate (7 5/8") advanced into the well bore. This setting of these different strings of pipe was started on 8-19-18 and completed on 10-9-18. Ron Burdick drilling supervisor for JKLM was at this inspection. No violations were noted at this inspection.
2811918 Primary Facility 2018-12-07 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) conducted a routine inspection of the JKLM Sweden Valley 104 well pad on 12/7/2018 @1115 hours. The weather was 23 degrees, overcast, 3-4" of snow was observed on the ground. Humidity 615 with winds WNW @12mph. Activity onsite consists of personnel removing the containment from the top hole rig activity, and placing it in onsite waste containers, in preparation for transition to the H&P flex rig.

No staining or melted spots were observed on the pad surface. No violations are noted at this time. I departed the site at 1225 hours.

2822391 Primary Facility 2019-01-02 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted a routine inspection of the JKLM Sweden Valley 104 Well Pad located in Summit Twp, Potter Co. on 1/2/2019 @1200 hours. The weather 32 degrees, cloudy, 78% humidity winds, S @5mph. The H&P 500 flex rig is onsite and is planning to drill 4 wells. Joe Huston, JKLM Company Man and Jared Jacobini, DEP WQS were present for the inspection.

Foam berms and garage was observed on the north fill slope and should be removed. No violations are noted at this time. We departed the site @

2822394 Primary Facility 2019-01-02 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted a routine inspection of the JKLM Sweden Valley 104 Well Pad located in Summit Twp, Potter Co. on 1/2/2019 @1200 hours. The weather 32 degrees, cloudy, 78% humidity winds, S @5mph. The H&P 500 flex rig is onsite and is planning to drill 4 wells. 2837956 Primary Facility 2019-02-05 Routine/Complete Inspection No Violations Noted At the time of this inspection this wells drilling was being completed. The H&P 500 Flex Rig and crew were advancing 5" production casing into the wells bore hole. The pipe will be set at a depth of 18,996' then will be cemented. Ron Hall the drilling consultant anticipated a time of 12:00 am for the completion of advancing the 5" production pipe. The hole was drilled to a TD (total depth ) of 19,043'. This drilling was started on 12-19-18. No violations were noted at this inspection.
