2633924 Primary Facility 2017-09-06 Routine/Partial Inspection No Violations Noted On 9/6/17 at 1228 hrs I attended a preconstruction meeting for the Headwaters 146 well site. JKLM representative, Jeff Long, J.L. Watts representative, John Watts, and New Growth Resources representative, Jim Fowler, were present. Penn E&R developed the Erosion and Sediment (E&S) control plan and J.L Watts is the construction contractor.

We reviewed the E&S drawing. Site drainage is to Cushing Hollow, which is designated as a High Quality stream. The access road will come off of Cushing Hollow Road. A general permit for a minor road crossing (GP-07-5329-17-001) was approved on 4/13/17. The GP-07 authorizes replacement of a stream culvert, which crosses Cushing Hollow at the access road entrance. An existing farm lane will be upgraded and serve as the access road for the site; total length of the access road will be 9,700 feet.

The site will consist of a well pad (approximately 350 feet x 500 feet) and two sediment traps. Sediment Trap 1 (ST-1) will be located beyond the southwest pad corner and Sediment
