2438037 Primary Facility 2016-01-05 Routine/Partial Inspection No Violations Noted Preconstruction meeting for the Sweden Valley 101 well pad. Logging operations to begin on 1/6/15. Site construction anticipated to begin after logging operations completed and will take approximately 3 weeks. TLT
2452377 Primary Facility 2016-02-18 Preoperation Inspection No Violations Noted Preoperational inspection. Spud notices for the 2HU - 5HU wells were received on 2/17/16 with an anticipated spud date of 2/19/16. Site has been constructed and stabilized. Perimeter E&S controls have been installed. TLT

2545908 Primary Facility 2016-12-19 Plugging(Includes Plugged/Mined Through) No Violations Noted On December 19, 2016 at approximately 10:30 am I arrived on JKLM’s Sweden Valley 102 pad after receiving Notice of Intent from the operator last week to plug and abandon (3) wells; 1HU (105-20848), 2HU (105-21868) both on the 102 pad and the 5HU (105-21858, on the 101 pad). All (3) wells being plugged are conductors only. There was a crew on site. I met with on-site representative to discuss operations on the pad and their plugging procedure. At the time of inspection the crew was digging out the first well; the conductors were cut and buried in the cellar after being buried. All the conductors are reportedly cemented in place outside the pipe and on bottom. After the cellars are dug out they’ll fill conductors with ~50’ cement on bottom (pour ready-mix) and fill with gravel from that point to surface. There are two conductors on this pad (1HU and 2HU) both 96’, and the 5HU on the 102 pad next door, set at 80’. Today they’ll dig out all three cellars in preparation for cementing operations which will likely
2546841 Primary Facility 2016-12-16 Routine/Complete Inspection No Violations Noted Routine inspection. The 2HU - 4HU wells have been drilled and stimulated. Production equipment has been installed at the site. The site was snow covered and frozen, therefore vegetative cover and E&S controls were not observed. TLT
2551436 Primary Facility 2017-01-09 Routine/Complete Inspection No Violations Noted
2609686 Primary Facility 2017-06-22 Routine/Partial Inspection No Violations Noted On 6/22/17 at 1345 hrs I conducted a routine inspection at the Sweden Valley 101 well site. It was overcast and 82 degrees F. Sean Bacha, Oil and Gas Consultant, was present at the site during the inspection.

The 2HU - 4HU wells were drilled in 2016 and are equipped for production. The 5HU well was spud, however, then plugged in December 2016. A new well permit application for the 6V well (105-21900) was received on 6/5/17.

I observed the E&S controls, production equipment and other materials staged on site. Grass is becoming well established on the topsoil stockpile and around the sediment traps; however, vegetative cover on the well pad fill slopes remains sparse. The sediment traps were observed (Photo #1). ST-1 (southern trap) held a minimal amount of water and had a field measured specific conductance of 209 micro Siemens/cm. The filter fabric had come off the rock filter and should be restaked (Photo #2). ST-2 (northern trap) was approximately half full and had a specific conductance of 73 micro Sie

2769269 Primary Facility 2018-08-23 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the JKLM Sweden Valley 101 Well Pad ESG29-105-15-0003 on 8/23/2018 @1130 hours. The weather was 70 degrees sunny, 58% humidity and winds WNW 13 mph.

The 2HU-4HU wells were drilled in 2016 and are currently in production. The 5HU well was spud, then plugged in December 2016.

The access road, well pad and associated controls were inspected. A compressor unit appears to have been installed on the SE corner of the well pad. Large puddles were observed on the pad surface from recent heavy rains. All puddles on teh pad surface measured <300mS/cm(Photo#3). The fluid in teh duck pond containment had a field measured conductivity of 300uS/cm. JKLM representatives were contact and a vac truck was dispatched to the site. The elevated conductivity fluid was removed.

A violation of 25 Pa Code 78a.56(a) is cited as IMMEDIATELY CORRECTED against the 2HU well API#105-21855 due to the remediation of the elevated conductivity fluid by a vac truck during the inspection.

I departe

2845928 Primary Facility 2019-02-27 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was a plugged well. This plugging was completed 12-21-16. The monument is a steel welded plate welded to the conductor 2 1/2' below ground surface. No violations noted at this inspection.
