2511451 Primary Facility 2016-08-16 Routine/Complete Inspection No Violations Noted Inspection of the Hardscrabble abandoned well in Pike Township, Potter County, conducted August 16, 2016 at 11:45 am. The well was shut in with cap threaded onto what I measured to be about 6 7/8” casing. The cap has pipe coming off of it with a ball valve that is shut in. One the other side of the ball valve the pipe is bullplugged. The casing is in decent shape with no holes, pitting, or visible discharge. There was an outer casing visible at ground level. I dug it up and measured it to be about 9 5/8” OD casing. There was water in the 6 7/8” annular space (9 5/8” x 6 7/8”). There was some minor bubbling in the water. I tested with an Altair 5x gas meter, barely got any reading. I covered the annulus with plastic and isolate the bubbling to my meter and it peaked at 3% methane after about 5 minutes. There was a vent pipe adjacent to the well coming up from the ground, which was bullplugged. This may be coming from the 9 5/8” (6 7/8” annulus) or from a larger diameter casing if there is one cut below ground
2521287 Primary Facility 2016-09-20 Routine/Complete Inspection No Violations Noted On September 20, 2016 at approximately 11:15 am I conducted a routine inspection of the Hardscrabble abandoned well in Pike Township, Potter County. I returned approximately 1 month since my last inspection (8-16-16) because my findings that day were inconsistent with previous reports in regards to brine/methane discharge. At the time of inspection; There was still minor bubbling in the annular space between the production casing and the surface casing. I tried to get a decent seal on the annulus by covering with plastic, sealing the plastic around the casing with a bungee cord, and slipped the hose for my gas meter under the cord and into the annulus right above the bubbling. The highest reading I was able to get with the Altair 5x gas meter was 36% LEL and 3% Methane after 5 minutes where it seemed to level off. There was no discharge of brine at this time.
2549649 Primary Facility 2017-01-03 Routine/Complete Inspection No Violations Noted Routine Inspection of the Hardscrabble abandoned well in Pike Twp. Potter County conducted January 3, 2017 at 1 pm. The (9 5/8” x 7”) annulus was completely full of water this time up to the top of the casing making the bubbling more apparent than last inspection was the water level was down a few foot in the annulus. It appeared to be fresh water in the annulus, no oil/brine noted. There was a steady rain and you could see where some of the fittings on the 7” well cap were leaking (gas). Gas reading above the bubbling was less than expected given the steady bubbling, ~7% LEL, down from last visit. Also noted during this inspection is the 7” casing is loose; I was able to move it by hand.
2585839 Primary Facility 2017-04-19 Routine/Complete Inspection No Violations Noted Routine inspection of the Hardscrabble Orphan Well conducted April 19, 2017 at 12 pm. The annulus was full of water to ground level with active bubbling and discoloration. The well had been marked with caution tape since my last visit.
2712698 Primary Facility 2018-03-27 Routine/Complete Inspection No Violations Noted A routine inspection of this abandoned well was conducted March 27, 2018. At the time of inspection, the 7" casing is capped and shut in at a valve. There area surrounding the well had high grass. No water or bubbling was seen or audible in the 7" annulus. Combustible gas was detected in this annulus at the time of inspection. The well is on the DEP plugging list and is tentatively scheduled for plugging this year or next.
2734796 Primary Facility 2018-05-24 Routine/Complete Inspection No Violations Noted A routine inspection of this leaking well on the DEP plugging list was conducted May 25, 2018 at 10:00 am. The well has been capped, shut in, and leaking from the annulus. I met with Alco Well Services on location who was contracted to blow down the well, remove the cap, install a swage, competent valve, and vent pipe on the casing. At the time of inspection, the contractor laid down containment and installed a competent valve on the piping off the well cap. The contractor gauged the well; 40 PSI. They proceeded to rig up iron from the well to a poly tank and opened the well. The well blew down in about 1 second. They removed the well cap, measured casing at 6 5/8", welded plate on collar with 2" pipe nipple to make a swedge, and installed a 2" valve with a candy cane vent. At this time, the gas leaking in the annulus was noticeably less than previously observed before blowing down and installing the vent.
2735744 Primary Facility 2018-05-29 Routine/Complete Inspection No Violations Noted A routine inspection of this well on the DEP plugging list was conducted May 29, 2018. At the time of inspection, a vent is installed on the 6 5/8" casing. There was no visible or audible venting, however, gas was detected at the vent with an Altair 5x gas meter. There was no bubbling or discharge observed in the annulus where the well had previously been leaking before the vent installation. No gas was detected in the annulus.
2737849 Primary Facility 2018-06-05 Routine/Complete Inspection No Violations Noted The Department was on location on June 5, 2018 to investigate this orphan well on the DEP plugging list. At the time of inspection, there was active bubbling inside the 6 5/8" from fluid approximately 5' below ground level. The bubbling in the annulus has ceased. A camera was run downhole to ~1200 (end of camera cable). The camera revealed end of 6 5/8" casing at ~544', and gas coming in from completed interval at ~928' in openhole section of the wellbore.
2741078 Primary Facility 2018-06-07 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this legacy well was conducted June 7, 2018. The well is on the Department's plugging list. At the time of inspection, the well has a 2" candy cane vent on 6 5/8" casing, venting a low volume of gas. Gas leaking in the annulus has ceased. A sample was taken from inside the casing. I spoke with the adjacent landowner, discussed access to the site and the general plugging operation, and received landowner consent. No obvious major issues were observed.
2747747 Primary Facility 2018-07-02 Routine/Complete Inspection No Violations Noted A routine inspection of this orphan well on the DEP plugging list was conducted July 2, 2018. At the time of inspection; the well has 6 5/8" casing made up to 2" candy cane vent. There was no audible or visible venting come from the vent. 100% gas was detected with an Altair 5x gas meter. No significant amount of pressure was felt at the vent. Bubbling in the annulus has ceased since vent installation. The well is tentatively scheduled to be permanently plugged later this year.
2756321 Primary Facility 2018-07-26 Routine/Complete Inspection No Violations Noted Arrived on location at 9:45 am for scheduled OOGM 18-1 plugging contract non-mandatory pre-bid meeting. At the time of inspection, the 6 5/8" casing is venting through 2" candy cane vent pipe. Bubbling in the annulus has ceased since vent installation. I discussed the known well information and contract tech specs with potential bidder. Left location at 10:30.
2763875 Primary Facility 2018-08-15 Routine/Complete Inspection No Violations Noted At the time of inspection, the 6 5/8" casing is venting through 2" candy cane vent pipe. Bubbling in the annulus has ceased since vent installation. No gas was detected in the annulus. 100% methane was detected inside the vent. A trace of gas was detected at the mouth of the vent. No pressure can be felt when placing hand over vent. There were no obvious signs of dead vegetation around the well. The field next to and leading up to the well has been cut since last visit. The well is scheduled to be plugged under a state contract this year.
2775402 Primary Facility 2018-09-13 Routine/Complete Inspection No Violations Noted At the time of inspection, the 6 5/8" casing is venting through 2" candy cane vent pipe. There was no obvious leaking observed in the 6 5/8" annulus. No leaking has been observed in the annulus since vent installation and there was no dead vegetation around the well at the time of this inspection. Venting can be observed at the mouth of the vent. The well is scheduled to be plugged under a state contract this year.

The well is being plugged under state contract OOGM 18-1. At the time of inspection, 6 5/8" casing is open and venting at 2" candy cane vent pipe. No gas was observed outside of casing. Downhole operations are tentatively scheduled for next week.
2786339 Primary Facility 2018-10-09 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this abandoned legacy well was conducted October 9, 2018. The well is being plugged under state contract OOGM 18-1. Plants & Goodwin Rig 10 is on location for final plug and abandonment. I met with the crew to discuss operations on the pad as well as depths, volumes, and procedure for today's work. They tagged bottom (previously unknown) at 2414'. They ran 4 1/2" casing with a packer at 561' (6 5/8" shoe @ 548) as a precaution for returned fluid since integrity of the 6 5/8" is unknown. Today they will be spotting the first 277' cement plug (50 sks) from bottom, gel uphole to gas zone at 938', then loading the hole with ~10# brine to kill well in preparation for cementing through this zone tomorrow. Water truck arrived for fresh mix water. They came online, saw good returns right away, and began loading hole with calculated volume of gel. Saw no leaks outside of 4 1/2" while pumping. After gel they will mix and pump cement slurry for bottom plug. Left location at approximat
2787333 Primary Facility 2018-10-10 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this abandoned legacy well was conducted October 11, 2018. The well is being plugged under state contract OOGM 18-1. Plants & Goodwin Rig 10 is on location for final plug and abandonment. I met with the supervisor to discuss operations on the pad as well as depths, volumes, and procedure for today's work. After I left location Tuesday, they successfully pumped bottomhole cement plug, killed the well, and plugged across gas zone @ 938'. There was reportedly no gas observed at surface after WOC overnight, and they tagged the plug at 862'. Today they are pumping the final cement plug to isolate the 6 5/8" (~548') casing shoe, with 1" workstring at 656' to spot plug. They finished mixing and pumping 18 sks without issue and are now pulling out of the hole. No gas was observed at surface and the plug appears to be balancing as they break connections. No obvious issues were observed.
2791267 Primary Facility 2018-10-18 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging, inspection of this plugged legacy well was conducted October 18, 2018. The well was plugged under state contract OOGM 18-1. Plants and Goodwin are off location following final plug and abandonment. At the time of inspection, all equipment has been removed from the site. The 6 5/8" x 2" candy cane vent pipe has been reinstalled along with a marker plate as a plugged well monument. There was no visible or audible venting observed at the vent. A field reading with an Altair 5x gas meter detected the presence of gas inside the vent. No gas was detected in the ambient air >1" away from the mouth of the vent. No significant amount of pressure could be felt at the vent. I attached a 0-160 PSI analog pressure gauge to the vent and monitored for 15 minutes; no pressure was observed on the gauge after 15 minutes. No bubbling was observed outside of surface casing. 6 5/8" surface casing was observed to be compromised before plugging at previously observed water level ~30'; and it is suspected
2791888 Primary Facility 2018-10-19 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging, inspection of this plugged legacy well was conducted October 19, 2018. Plants and Goodwin are off location following final plug and abandonment operations as part of state contract OOGM 18-1. At the time of inspection, all equipment has been removed from the site. The 6 5/8" x 2" candy cane vent pipe has been reinstalled along with a marker plate as a plugged well monument. There was no visible or audible venting observed at the vent. A field reading with an Altair 5x gas meter detected 100% methane at the vent. No significant amount of pressure could be felt at the vent. Periodic bubbling was observed in the 6 5/8" casing annulus. Conditions have seemingly not improved since last inspection; additional follow-up will be conducted on Monday.
2792778 Primary Facility 2018-10-22 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, final post-plugging inspection of this plugged legacy well was conducted October 22, 2018. Plants and Goodwin are off location following final plug and abandonment of the well under state contract OOGM 18-1. At the time of inspection, the well has an open 6 5/8" x 2" candy cane vent with marker plate installed on the casing as a plugged well monument. There was no visible or audible venting observed at the vent. A field reading with an Altair 5x gas meter detected methane inside the vent pipe. No gas was detected in the ambient air less than 1" from the mouth of the vent. Additionally, the bubbling in the 6 5/8" casing has ceased; no active bubbling was observed during the inspection. I attached a u-tube manometer, capable of measuring pressure < 1 PSI, to the well directly (no orifice plate) and monitored for 30 minutes. The fluid in the manometer rose to approximately 10 inches of water differential (.36 PSI) in the first minute and remained constant, showing no increase, during the duration of
2797741 Primary Facility 2018-11-02 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this DEP Plugged well was conducted November 2, 2018. The well has a 2" vent pipe monument. No venting was visible or audible during the inspection. The fluid in the annulus was stagnant; no bubbling was observed. No obvious major issues were observed.
2806733 Primary Facility 2018-11-21 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging inspection of this DEP-Plugged legacy well was conducted November 21, 2018. At the time of inspection, the location was snow covered. There is a 2" vent pipe installed on 6 5/8" casing. There was no visible venting or discharge. No bubbling or discharge was observed in the annulus. The snow surrounding the well was clean; no evidence of discharge. No obvious issues were observed.
2819208 Primary Facility 2018-12-26 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging, inspection of this DEP-plugged legacy well was conducted December 26, 2018. At the time of inspection, the location was damp and partially snow-covered. No bubbling, discharge, or evidence of discharge was observed on location. The well has a 2" candy cane vent pipe erected as a plugged well monument. There was no visible or audible venting observed. The annulus has standing fluid approximately 2' below grade. No bubbling was observed in the annulus. The immediate area surrounding the well is partially snow-covered; no evidence of discharge was observed. No obvious issues were observed during this inspection.
2830666 Primary Facility 2019-01-23 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging, inspection of this DEP-plugged legacy well was conducted January 23, 2019. The well has a 2" candy cane vent pipe erected on 6 5/8" surface casing as a plugged well monument. The location was covered with ice and snow. The ice/snow surrounding the base of the well was clean; no evidence of discharge. There was no visible or audible venting observed. No obvious issues were observed at this time.
