2000004 Primary Facility 2011-09-01 Complaint Inspection No Violations Noted A site inspection was conducted on Sept. 1, 2011 at 08200 in response to a complaint received on Aug. 30, 2011. The complainant observed a greenish colored liquid in the road ditch coming from the well site location. The site ID sign, E&S control plan and perimeter BMPs were observed. The inspection report for the 1H well (1999989) document the finding of the inspection. The inspection was concluded at 0913.

2301941 Primary Facility 2014-09-04 Site Restoration No Violations Noted The Department inspected the Penn Virginia O&G NE Timberlands site, permit #ESX10-105-0024, in Coudersport Borough, Potter Co., @ 1030 hours on 9/4/14. The weather was 74 degrees and sunny. The restoration plans were located in the mailbox onsite. All temporary BMP’s appear to have been removed. Areas designated as revegetated on the restoration plans appear to have >70% vegetation. Conductivity readings were taken from areas of standing water of the pad. Readings were between 45µs/cm-97µs/cm. Well tags were visible, but not affixed to the well heads. Wooden debris, pallets and production equipment should be removed from the restored pad.

2572333 Primary Facility 2017-03-13 Routine/Complete Inspection No Violations Noted On 3/13/17 at 1000 hrs I conducted a routine inspection of the NE Timberlands well site. It was partly sunny and 18 degrees F. No one was present during the inspection.

Final restoration of the well site was completed last summer following plugging of the 1H well on 6/14/16. The site was graded to approximate original contours and hydroseeded; these observations were documented in my 7/14/16 inspection report.

During this inspection, the site was snow covered and permanent stabilization could not be verified. The two concrete-weighted posts observed during the last inspection remained on site near the entrance. As per Section 3216(d) of 58 Pa.C.S. Sections 3201 - 3274 (relating to Development) ("2012 Oil and Gas Act) all production or storage facilities, supplies and equipment shall be removed and the well site restored within nine months after plugging a well. The weighted posts should be removed as part of the final restoration.

2628631 Primary Facility 2017-08-23 Site Restoration No Violations Noted On 8/23/17 at 1335 hrs I conducted a restoration inspection of the NE Timberlands well site. It was partly cloudy and 75 degrees F. No one was present during the inspection.

Final restoration of the well site was completed following plugging of the 1H well on 6/14/16. The site was graded to approximate original contours and hydroseeded. The site is permanently stabilized as per 25 Pa Code Section 102.22(a).

An electronic copy of the Well Site Restoration Report (WSRR) was received on 8/14/17; the report was reviewed on 8/23/17 and found to be administratively complete.

No violations were cited during this inspection. I concluded the inspection at 1445 hrs. TLT
