The site was determined to meet the requirements of 25 Pa Code §102.22(a)(2) (i) Permanent Stabilization. A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was observed. The approval of the Notice of Termination is recommended. The Well Site Restoration Report is being approved. No new violations were cited during this inspection. -MRN

2571905 Primary Facility 2017-03-13 Routine/Complete Inspection No Violations Noted On 3/13/17 at 1210 hrs I conducted a routine inspection of the Risser well site. It was overcast and 25 degrees F. No one was present during the inspection.

Final restoration of the well site was completed following plugging of the 1H and 2H wells in August 2016. The site has been graded to original contour and seeded and mulched. Above-ground monuments for the plugged wells were not observed in the restored area.

The access road, which was a pre-existing farm lane, was left in place. Some equipment remains at the site, including a large vented tank and other miscellaneous items. As per Section 3216(d) of 58 Pa.C.S. Sections 3201 - 3274 (relating to Development) ("2012 Oil and Gas Act) all production or storage facilities, supplies and equipment shall be removed and the well site restored within nine months after plugging a well. The tank and other miscellaneous items should be removed as part of the final restoration.

2628389 Primary Facility 2017-08-23 Site Restoration No Violations Noted On 8/23/17 at 1052 hrs I conducted a restoration inspection of the Risser well site. It was mostly sunny and 65 degrees F. No one was present during the inspection.

Final restoration of the well site was completed following plugging of the 1H and 2H wells in August 2016. The site has been graded to original contours and meets the permanent stabilization requirements found under 25 Pa Code Section 102.22(a). The access road, which was a pre-existing farm lane, was left in place. Since the last inspection on 3/13/17, the equipment that remained on site has been removed.

An electronic copy of the Well Site Restoration Report (WSRR) was received on 8/14/17; the report was reviewed on 8/23/17 and found to be administratively complete. No violations were cited during this inspection. I concluded the inspection at 1140 hrs. TLT
