2058596 Primary Facility 2012-04-09 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected On April 9, 2012 an inspection at the Dunn pad in Allegany Township, Potter County revealed an unreported spill near the steps of the secondary containment of the production fluids storage tank. The puddle exhibited an elevated specific conductance (over limits). The cause of the release could not be determined. Sean Mahaffey of Penn Virginia was informed of the situation via email and phone form the site. Sean responded and stated that a vacuum-truck had been dispatched to the site and the cause of the release will be investigated and the results provided to the Department. The release is a violation of 25 Pa Code §91.33(a) (failure to report a spill), 35 PS § 6018.301 SWMA (Management of Residual Waste) and 25 Pa. Code, §78.57(a) (Failure to adequately control, store and dispose of a production fluid). The puddle was vacuumed up the same day, violations will be noted as immediately corrected. MRN

The site was determined to meet the requirements of 25 Pa Code §102.22(a)(2) (i) Permanent Stabilization. A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was observed. The approval of the Notice of Termination is recommended. The Well Site Restoration Report is being approved. No new violations were cited during this inspection.

Final restoration of the well site was completed following plugging of the 1H well on 8/21/16. The site has been graded to original contours and seeded and mulched.

To date, a Well Site Restoration Report (WSRR) for the 1H well has not been received. The operator should submit the WSRR as per 25 Pa Code Section 78a.65(f), which states that within 60 calendar days after post-plugging restoration the operator shall submit a restoration report to the Department.

No violations were cited during this inspection. I concluded the inspection at 1400 hrs. TLT

2628461 Primary Facility 2017-08-23 Site Restoration No Violations Noted On 8/23/17 at 1248 hrs I conducted a restoration inspection of the Dunn well site. It was mostly sunny and 70 degrees F. No one was present during the inspection.

Final restoration of the well site was completed following plugging of the 1H well in August 2016. The access road has been removed and the site has been graded to original contours and permanently stabilized as per 25 Pa Code Section 102.22(a).

The Well Site Restoration Report (WSRR) was submitted electronically on 8/14/17; the report was reviewed on 8/23/17 and found to be administratively complete. No violations were cited during this inspection. I concluded the inspection at 1305 hrs. TLT
