The well was drilled and awaiting fracing. There was no fence around the pit. This is a violation of 25 PA Code §78.56 (a)(3) ¿ Pits and tanks shall be designed, constructed and maintained to be structurally sound and reasonably protected from unauthorized acts of third parties.

The perimeter of the site, Erosion Control Plan and Erosion control BMPs were observed. The spill adjacent to the well was cleaned up, the disturbed areas seeded and mulched and the fence around the pit has been erected. All violations have been addressed and corrected. 2633197 Primary Facility 2017-09-06 Site Restoration No Violations Noted On 9/6/17 at 1341 hrs I conducted a restoration inspection of the Butler #1 well site. No one was present at the site. It was overcast and 67 degrees F. The Butler #1 well was drilled by Allegany Enterprises in July 2011. The well was tagged. The site is restored and permanently stabilized as per 25 Pa Code Section 102.22(a). The Well Site Restoration Report (WSRR) was received on 5/4/17; it was reviewed and acknowledged on 9/6/17. According to the WSRR, an encapsulated drill cuttings pit is located 22 feet west of the wellhead. No violations were cited during the inspection. I departed the location at 1400 hrs. TLT

The wellhead assembly all fittings and flow lines were tested with a gas meter. Results of this testing was no gas detected on the wellhead assembly or the well area.

2833140 Primary Facility 2019-01-29 Routine/Complete Inspection No Violations Noted At the time of this inspection this well appeared to be in production. Well is an approximate 10" casing out of the ground reduced to an approximate 7" casing that is threaded into the larger casing. Threaded onto the smaller casing is a tee (t) style fitting that is approximately 5" x 2" x 2". Out of one side of the tee fitting is an approximate 2" valve that is open. On the other side of the tee fitting is an approximate 2" valve that is attached to the flow/ production line that goes into the ground then goes to production equipment. No violations noted at this inspection.
