2032996 Primary Facility 2012-01-09 Routine/Complete Inspection No Violations Noted An inspection was conducted on 1/9/12 at the DCNR Tract 001 - Pad H site in Sweden Township, Potter County. The site ID sign, ESCGP, and perimeter of the site were observed. The ground was frozen and mostly snow covered. No evidence of accelerated erosion was observed. Where visible, the slopes appeared stable and vegetated. In several areas on the pad surface, there were small amounts of a balck, shiny rock-like material spilled. Doug Kepler was contacted and provided pictures of the spilled material. Mr. Kepler informed the Department that the material is used for sandblasting and had been cleaned up the morning of 1/11/12. Recent weekly inspection records for this site were not in the mailbox for this site, this issue needs to be addressed. No new violations were cited during this inspection. - MRN
2179504 Primary Facility 2013-07-01 Routine/Partial Inspection No Violations Noted A routine inspection of the DCNR 001 1V 50258 Pad in Sweden Township, Potter County was conducted on July 1, 2013 All E&S controls remain in place and no runoff was visible leaving the site. Several puddles on the pad surface were screened for elevated conductance; no elevated reading were observed. The 1V well was drilled from 7/1/2010 to 7/10/2010, the site should be restored. No violations were cited during this inspection.

2582583 Primary Facility 2017-04-10 Routine/Complete Inspection No Violations Noted On 4/10/17 at 1445 hrs I conducted a routine inspection at the DCNR 001 1V 50258 monitoring pad. It was mostly sunny and 73 degrees F. No one was present during my inspection.

The 1V well was drilled in 2010 and plugged in 2013; a vent pipe and monument mark the plugged well. The monitoring pad has been reclaimed, including the access road. Vegetation is becoming established, however, the former pad is rocky and uniform cover has not yet been achieved. Tree plantings were done as part of the restoration and numerous tree tubes were observed on the ground. Silt sock remains in place along the east side of the well pad.

A Well Site Restoration Report (WSRR) should be submitted as per 25 Pa Code Section 78a.65(f).

No violations were cited during this inspection. I concluded the inspection at 1505 hrs. TLT

2639908 Primary Facility 2017-09-26 Site Restoration No Violations Noted On 9/26/17 at 1101 hrs I conducted an inspection at the DCNR 001 1V 50258 monitoring pad. It was mostly sunny and 80 degrees F. No one was present during my inspection.

On 9/22/17 Seneca Resources electronically submitted a post-drilling Well Site Restoration Report (WSRR) for the 1V well; the WSRR was reviewed on 9/26/17. The ESCGP for the DCNR 001 Pad H site and the 1V well pad has not yet been terminated.

The 1V well was drilled in 2010 and plugged in 2013; a vent pipe and monument mark the plugged well. The monitoring pad and access road have been reclaimed to near original contours and the site is permanently stabilized as per 25 Pa Code Section 102.22(a).

The WSRR will be acknowledged upon return to the office. No violations were cited during this inspection. I concluded the inspection at 1318 hrs. TLT

No one was present during the inspection. The 1V well was drilled in 2010 and plugged in 2013; a vent pipe and monument mark the plugged well. The monitoring pad and access road have been reclaimed to near original contours and the site is permanently stabilized as per 25 Pa Code Section 102.22(a). The ESCGP for the DCNR 001 Pad H site and the 1V well pad has not yet been terminated. The WSRR was acknowledged on 9/27/2017. No violations were noted during this inspection.
