The 4H (105-21623) and 5H (105-21624) wells are drilled. The 1H(105-21656) well is not drilled, but the permit is active. This permit is not listed on the site ID sign and should be added. The site is stable and well vegetated. No puddles screened exhibited elevate conductivity. The 1H (105-21620), 2H (105-21621), 3H105-21622, 6H (105-21625), 6H (105-21657), 2H (105-21741) and 3H(105-21742) have been reported as Operator Reported Not Drilled (ORND). No issues were noted and no violations were cited during this inspection.

The 4H & 5H wells have been drilled and are equipped for production. Site is permanently stabilized. Impoundment was 3/4 full and frozen over. Site has not been restored. SWEPI intends to submit a request in 2016 to extend the restoration requirements for the site. No violations cited. TLT

2751345 Primary Facility 2018-07-12 Routine/Complete Inspection No Violations Noted I conducted a ROUTINE inspection of the SWEPI LP-Marshlands Play Mitchell A 903 Pad on 7/12/18 at 1330 hrs. The weather was 74 degrees, sunny and 38% humidity, winds NNE 5mph. No one was present during the inspection.

I observed the access road, well pad and freshwater impoundment. Two wells, the Mitchell 903 4H and 5H have been drilled. Both wells appear to be in production. Produced water is stored in a Highland production tank, which has built-in secondary containment. The tank was last inspected 7/6/2018.

The impoundment is located adjacent to the well pad. A protective fence and adequate freeboard were observed.

The well site has been restored and post-construction stormwater management (PCSM) BMPs have been installed. The ESCGP was terminated 8/7/2017.

No violations were cited during this inspection. I departed the site at 1430 hours
