2155310 Primary Facility 2013-04-10 Routine/Partial Inspection No Violations Noted An inspection of the Pine Hill West Pad B in Sylvania Township, Potter County was conducted on April 10, 2013. The last self-inspection conducted on the behalf of PGE was on 4/3/2013 and no issues were noted. The 2383 well has been drilled, but is not being produced. The 2382 well has been spudded, but not drilled. The site was stabilized and no puddles screened exhibited elevated conductance. No violations were cited during this inspection.
2593494 Primary Facility 2017-05-10 Routine/Complete Inspection No Violations Noted On 5/10/17 at 1248 hrs I conducted a routine inspection at the Pine Hill well site. It was overcast and 60 degrees F. No one was present during my inspection.

The site includes an access road, the well pad and an impoundment. Two wells, the Pine Hill 2382 and 2383, have been drilled and were placed on inactive status on 12/11/13. Drilling of the wells was completed in November 2009; there have been no new well permits issued for the site since that time. It did not appear that the well pad had been restored and the site remains at full size (approximately 300 feet x 475 feet). Also, it did not appear that post-construction stormwater management (PCSM) BMPs have been installed. Interim restoration of the well site should be completed as per 25 Pa Code Section 78a.65(a)(1).

On 8/13/15, PGE submitted a Restoration Compliance Report to the Department. The Plan/Schedule for the Pine Hill 2382 site stated that an updated PCSM plan would be submitted to the Department for review and approval within two months. To

2730910 Primary Facility 2018-05-14 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in Regulatory Inactive Status.
2740772 Primary Facility 2018-06-05 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted a routine inspection of the Pennsylvania General Energy – Pine Hill 2382 (Pine Hill Sylvania Pad B) located in Sylvania Twp., Potter Co. on 6/5/2018 @ 1100 hours. The weather was 65 degrees and overcast.

PGE had set the conductor for the Pine Hill Sylvania, Pad B 2740 well (API#105-21910) on the previous day. Cement was observed at the surface surrounding the conductor pipe. Two existing wells were observed onsite 105-21589 and 105-21590, that are listed as in Regulatory Inactive Status (RIS). John Boardman stated PGE plans to bring a Patterson rig to the site in July to drill the lateral portion of the 2740 well. Waste Management was onsite removing the cuttings, from the top hole portion of the well, that were stockpiled in a waste container adjacent to the well.

Onsite PGE representative Cory Dean stated they were in the process of resurfacing the pad. Stone was observed stockpiled along the northern side of the pad. Recent earth disturbance activities

2762318 Primary Facility 2018-08-08 Routine/Complete Inspection No Violations Noted At the time of this inspection this wells status was Regulatory Inactive Status. No violations were noted at the time of this inspection.
2762739 Primary Facility 2018-08-07 Follow-up Inspection No Violations Noted The DEP conducted a Follow-Up Inspection of the PGE – Pine Hill 2382 (Pine Hill Sylvania Pad B) located in Sylvania Twp., Potter Co. on 8/7/2018 @1100 hours. The weather was 77° overcast, 71% humidity. Korey Starkey, DEP Env Trainee and Zach Fadurko, PGE were present onsite for the inspection.

The access road, well pad, containment and onsite inspection logs were inspected. The Patterson PeakeRig 83 is currently onsite drilling the Pine Hill 2740 (API#105-21910), a Utica well. At the time of the inspection the rig was at 11,036’. Mr. Fadurko stated drilling is utilizing freshwater from the onsite impoundment along with collected rainwater being stored in tanks onsite.

We departed the site at 1300 hours.

2802436 Primary Facility 2018-11-09 Administrative/File Review No Violations Noted Admin inspection for purpose of linking to a plugging COA.
2825801 Primary Facility 2019-01-11 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in Regulatory Inactive Status. An inactive extension was granted for this well and this extension will expire on 12-11-19. The annual inactive casing pressure test on this well was conducted on 1-8-19. Results submitted for the testing of this well were noted that the casing pressure was 3875 psi (pounds per square inch). The annular vents are not vented to surface at the well and are hard piped (plumbed) from the well to a production/ waste tank on the pad. Results of the vent testing submitted were 0 psi on the vents. A mechanical gauge on top of the wellhead assembly verified the casing psi of 3875 psi that was submitted. No violations were noted at this inspection.
