2152733 Primary Facility 2013-04-01 Routine/Partial Inspection No Violations Noted An inspection of the Pine Hill 2369 well site in Wharton Township, Potter County was conducted on April 1, 2013 in response to Well Site Restoration Reports received by the Department. The last self-inspection conducted on behalf of PGE was on 3/27/2013. This well has been plugged. The site has been re-graded to original contours and temporarily stabilized with mulch. The site was determined to be temporarily stabilized, but does not meet the requirements of 25 Pa Code §102.22(a)(2) (i) Permanent Stabilization at this time. A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was not observed. A follow up inspection will be conducted at a later date to verify the site has been permanently stabilized and the E&S controls have been removed. The Well Site Restoration Reports will not be approved until that time. No violations were cited during this inspection.

This well has been plugged. The site has been re-graded to original contours and temporarily stabilized with mulch. The site was determined to meet the requirements of 25 Pa Code §102.22(a)(2) (i) Permanent Stabilization at this time. A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was observed. On 10/18/13 the operator sent photos verifying the E&S controls have been removed. The Well Site Restoration Report will be approved at this time. No violations were cited during this inspection.
