2753039 Primary Facility 2018-07-18 Routine/Complete Inspection No Violations Noted I received an email from the landowner where this well was plugged under contract OOGM 13-3. The email stated the water was pooling in the area of where the well is and there is a sheen on the water. The area in question is a wet patch of ground that extends from the well to a point approximately 20 away. I checked slam bar holes for methane but did not receive a reading for methane. Water was not pooled at the time of inspection, so no sheen was observed. No oil staining was noticed at the time of inspection. There is a drain installed in the yard in close proximity to the well that runs to North Cole Creek. At the drain discharge there is iron accumulation. A metal detector picked up several areas that appear to be lines under ground. The lines could be drains or old pipelines. The home is in the bottom of a valley and the lower yard is an extremely wet area. The lower yard is not maintained at this time.

I notified the landowner of my findings and asked her to check the sheen to see if it breaks or pull
