CC Forbes rig onsite preparing for plugging operations. Met with company man Daryl Jolly. Location was quiet at time of inspection -- waiting on an accumulator, flare stack , and gas buster before commencing operations. Surface casing 9-5/8”, depth: 846’ – planning to first evaluate casing integrity and cement bond. Halliburton will be doing the logging, Universal the cementing. Location was tidy.
2393234 Primary Facility 2015-07-24 Routine/Complete Inspection No Violations Noted Weather Sunny and 75°. Arrive on Location by myself. I was greeted by Darrell Jolley at the edge of location and we discussed what was on going on location. Rig crew was busy setting up the flare tank and gas buster that had recently arrived on location. After testing the second accumulator it failed and a third was on order, arriving by approximately 3pm. Mr. Jolley said he expected to begin operations to clean the hole and determine the depth of the casing seat by Saturday Morning/Afternoon
2393256 Primary Facility 2015-07-26 Routine/Complete Inspection No Violations Noted Remarks: Weather Sunny and 85°. Met Darrell Jolley on location with Tom. They informed me of their previous day’s work. They RIH with an 8 ¾ bit to the casing shoe with no obstructions then they ran in the hole with a scraping tool to the casing shoe also with no obstructions. Halliburton logged a CBL on the hole for them which they provided me a copy of. The CBL showed good bond the length of the casing. They were next preparing to RIH with the 8 ¾ bit to see how far down they could go. Location was clean and no Violations were noted.

2394607 Primary Facility 2015-08-02 Routine/Complete Inspection No Violations Noted Weather 77° and Sunny. I arrived on location at 1400 on Sunday the 2nd. When I arrived on location I observed the rig torqueing out something solid. I spoke with Darrell Jolley and he said that they are at 6880’ and are having a lot of trouble getting any further. Some small metal cuttings were observed in the returns. They had not experienced any gas in the returns since mud was introduced to the hole. At this time they were waiting on approval from the Department to call 6800 attainable bottom so they could begin cementing operations. No violations were noted.
2395385 Primary Facility 2015-08-04 Routine/Complete Inspection No Violations Noted Weather 73° and Partly Sunny. When I arrived rig was waiting to tag plug @ 3250’. The samples from the other plugs were all hard. Tagged plug @ 3350’ with 2000 lbs. Darrell Jolley and Tom Drewett were asking for approval for that TOC. I confirmed with Mike Panettieri that 3350 was ok and rig could proceed with final plugs. Universal set a 250’ plug at 700’ and planned to WOC until 1930. I arrived back OL at 1900 and rig began to RIH. Plug was tagged at 709’ with 1000 lbs. When I departed rig was preparing to set final 100’ plug from 250’ to 150’, and begin rig down. No Violations were noted.
2396571 Primary Facility 2015-08-06 Routine/Complete Inspection No Violations Noted Sunny 68° Arrived on location and Rig was in the process of moving out. BOP was still installed on wellhead but was open to the atmosphere. Used Altair gas meter on open port and detected 0% Methane. Operator to photograph monument with ID plate attached and submit with plugging certificate.
2411030 Primary Facility 2015-06-30 Routine/Complete Inspection No Violations Noted Daniel Harvey, Erin Gocek, and I (all DEP) arrived at 08:55 to inspect the site. At the time of the inspection, it was raining and construction was put on hold until conditions improve. The access road was upgraded with some small stone but was showing significant wear due to truck traffic. On the Thompson (tree farm) property the road was being built along the edge of the fields. The access road between the existing fields and the Ambrose Ging well pad was been cleared. A portion of the proposed well pad has been cleared and water features flowing onto the pad have been dammed (plastic and sandbags) to keep as much water off the pad as possible. Bubbling in the area of the well is much reduced since the last site inspection on 4/2/2015. Overall, the conditions on the site present some challenges in terms of current geography and the wet conditions. No violations
2411032 Primary Facility 2015-09-24 Routine/Complete Inspection No Violations Noted Routine Inspection 9/24/2015.
2465529 Primary Facility 2016-03-16 Routine/Complete Inspection No Violations Noted The well site has been restored and the wetland regeneration looks good. The access road has some evidence of minimal erosion of the limestone surface. In addition, the E&S controls along the access road contain sediment that should be removed so they remain effective. Overall, the site looks good but I recommend maintaining E&S controls since vegetation is not fully established on the entire site. No Violations Noted.
2497570 Primary Facility 2016-06-30 Routine/Complete Inspection No Violations Noted Routine Inspection; 6/30/2016.

The well pad that was reconstructed around the Ambrose Ging well has been reclaimed/restored and the wetland features left from the original pad have also been restored as noted in the 9/24/2015 inspection. Vegetation is growing well for the most part throughout the site. Stream bank stability on the stream crossing has deteriorated and I recommend re-stabilizing the banks. I also recommend stabilizing the newly installed turn-outs or monitoring to make sure stability of the material is not an issue. An infiltration berm constructed of silt sock has been installed since the last inspection. No Violations Noted.
