Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2798708 Primary Facility 2018-11-06 Drilling/Alteration No Violations Noted Surface hole rig drilling conductor for permitted wells. All cuttings being disposed of properly. All containers within secondary containment. No issues noted.
2804373 Primary Facility 2018-11-13 Drilling/Alteration No Violations Noted Weather was 43° and cloudy at time of inspection. There were no Alta representatives on location at time of inspection. Well was a spud conductor only. Conductor casing was set and cemented in place. The cellar rings were in the process of being installed at time of inspection. No violations were noted at time of inspection.
2806700 Primary Facility 2018-11-19 Routine/Complete Inspection No Violations Noted Weather was 35° and cloudy at time of inspection. There were no Alta representatives on location at time of inspection. Well was a spud conductor only. Conductor casing was set and cemented in place. The casing had a steel plate welded over the top at time of inspection. Well cellar rings were installed. No violations were noted at time of inspection.
2824640 Primary Facility 2019-01-09 Routine/Complete Inspection No Violations Noted I met Bill Mitchell of ARD when I arrived. H&P rig 225 on location, running 9 5/8 casing in the 1H well. Containments appeared to be in good condition. On the south side of the pad, I noted several small puddles with a blue tint that had a soapy foam on them. The rig was being washed at the time, making it likely that this was rig wash water escaping containment. I also noted a pump just off of the east side of the containment that had a light spattering of a red substance frozen in the ice next to it. When I showed these areas to Bill, he noted that the red substance was likely diesel fuel, and told me he would have both areas cleaned up. I also pointed out to Bill a pallet of bags of citric acid that were not in containment, and he told me he would see to it that they were placed in containment. Some water (1-2") had built up around the edges of the containment, and Bill said he would have these areas vacced.
2827889 Primary Facility 2019-01-14 Routine/Complete Inspection No Violations Noted Weather was 16° and sunny at time of inspection. Met with Alta representative Bill Mitchell on location. This well recently had its surface and intermediate casing drilled and cemented. Reviewed the daily activity reports and the cement job logs from each string and no discrepancies were noted at time of inspection. Well was closed in with a nightcap and covered with rig matting at time of inspection. The rig was drilling the lateral section of the 4H well. Once the 4H is completed the rig intends on moving to the 3H to complete it.
2839880 Primary Facility 2019-02-13 Routine/Complete Inspection No Violations Noted Weather was 28° and cloudy at time of inspection. Met with Alta representative Bill Mitchell on location. The rig was currently in the process of drilling the 8 3/4" hole for the production casing. Discussed how drilling was progressing and upcoming cementing operations. Rig is slated to move over to the Elbow Fish and Game tract after completing the last 2 wells on this pad. Conducted a walk around inspection of location and no violations were noted at time of inspection.
2840572 Primary Facility 2019-02-14 Routine/Complete Inspection No Violations Noted I met RD Mann when I arrived. H&P 225 rig on site, drilling at 14800' on the 2B well. Snow and ice cover on containment, but berms appear to be in good condition. The area under the rig shakers had an accumulation of oily water, as did the area behind the mud tanks. I recommended to RD that he have these areas vacced. Chemical containment was sound, although snow had accumulated in it.
2843193 Primary Facility 2019-02-22 Routine/Complete Inspection No Violations Noted Weather was 29 degrees and sunny. Met with Alta representative R.D. Mann. While drilling rig decided to do a bit trip from a TD of 17097'. While pulling drill pipe it packed off and became stuck at 13935'. After several attempts to free the pipe, they decided to back off at 7208'. The successfully backed off the drill pipe and began to fish. They are assuming they separated their motor downhole. If attempts to fish are unsuccessful they plan to plug back and sidetrack. No violations noted at time of inspection.
2843901 Primary Facility 2019-02-25 Routine/Complete Inspection No Violations Noted Weather was 24 degrees and sunny. Met with Alta representative R.D. Mann on location. Rig was unsuccessful in fishing the drill pipe and BHA out of the hole. They decided to plug back and sidetrack. The 5' drill pipe is back off at 7266'. Sunday morning they set 2 cement plugs. The first 500' plug was set from 6296' to 5796'. They then pulled up to 5611' and circulated the hole. They then set a 700' plug from 5695' to 4995'. They used 17.2 lbs. cement for the 2 plugs. At time of inspection rig was preparing to trip back in the hole to begin drilling their new lateral. No violations noted at time of inspection.
2849958 Primary Facility 2019-03-07 Routine/Complete Inspection No Violations Noted Weather was 25 degrees and sunny. Met with Alta representative Bill Mitchell on location. Rig had recently set a plug to sidetrack around a failed fish. Viewed the cement job log from the cement plug. Top of the kickoff plug was set at 5800'. Rig successfully kicked off and were drilling their new lateral. No violations noted at time of inspection.
2852918 Primary Facility 2019-03-13 Routine/Complete Inspection No Violations Noted Weather was 30 degrees and sunny. Met with Alta representative Bill Mitchell on location. Rig had recently drilled and set new lateral to a TD of 16677'. Rig set and cemented casing at a depth of 15816'. Reviewed cement job log and no discrepancies noted at time of inspection. Well was currently shut in with a night cap. No violations noted at time of inspection.
