At the time of the inspection, one well was in production. The annular vent is plumbed to a poly tank. The production tank is in containment and is in good condition. PCSM was previously implemented and a 2-year extension was approved on 2/24/2016. The 2-year extension expires on 2/24/2018. I recommend evaluating the well site restoration plans. No violations noted.

2758851 Primary Facility 2018-08-01 Routine/Complete Inspection No Violations Noted Weather was 73° and cloudy at time of inspection. Met with Chief representative Floyd Roberts on location. Well was recently cemented to TD and shut in. Cudd was on location beginning to rig up on the well to complete Toe prep operations. Annular spaces were visible from surface and cement was visible. No violations were noted at time of inspection.
2784370 Primary Facility 2018-10-03 Routine/Complete Inspection No Violations Noted Site was in flowback phase when I arrived; 2 temporary separators and heaters were hooked up to the wells. The site had a large number of puddles, both on and off containment. These all showed low conductance (.18-.33 mS/ cm). Water had also built up under the liner, creating small spots where the liner was pushed up by the water. Berms had been removed from the sitewide containment, but no equipment remained on that liner. The containments for the Baker tank batteries on site appeared to be in good condition; however, on the outside of the berms for these containments, the liner was stretched such that it was not making contact with the ground for 1-3', increasing the possibility of a tear. The site has an existing pipeline, and the wells will be put directly to sales after flowback is complete.
2793159 Primary Facility 2018-10-23 Routine/Complete Inspection No Violations Noted Three wells in flowback. Site remains in flowback with production going to pipeline for sale. Mobile GPUs and sand separators situated near well heads. All separators within temporary containment. Many (20+) baker tanks within containment. No leaks. No conductivity above 300 uS/cm inside containments.
