After entering the site we talked to Tom Umstead (Hawbaker) who was managing the site. We discussed the seeding and stabilization as well as the infiltration berm construction and stabilization. At the time of the inspection, the main cut/fill of the pad area was being constructed. A full spread of equipment was working and a water truck was applying water for dust suppression throughout the main work area and especially the haul truck path from the cut side down to the fill slope and back. Overall, the BMP's are being installed and applied properly. I appreciate the care used in installing the infiltration controls, stabilizing progressively (as the project continues), and the active use of dust control measures. Overall, the site is in good condition. No violations noted.

2763014 Primary Facility 2018-08-09 Preoperation Inspection No Violations Noted Pre-operational inspection; 8/9/2018.

I entered the site at 17:05 and talked to Tom Umstead (Hawbaker). Work was ongoing to establish the cut/fill and under-drain. The cut side was being drilled to be shot at the time of the inspection.

Recently, heavy rains far in excess of any design storm rate/volume have impacted the area and this site shows signs of some previous impacts. Sediment on perimeter E&S controls indicates that they have overtopped. An 8" HDPE pipe was used as a drain at one location where the silt sock overtopped to bypass the perimeter E&S controls. In addition, evidence suggest the northernmost bio-retention berm filled and breached as evidence by the volume of sediment behind the E&S controls. The E&S issues described above have been corrected onsite and the dead-end road immediately north and east of the fill slope (outside the LOD) has been recently rock-lined, and re-vegetated. 2773829 Primary Facility 2018-09-12 Routine/Complete Inspection No Violations Noted Weather was 67° and raining at time of inspection. Met with Seneca representative Scott Barr on location. Conductor was set and cemented the previous day. Good returns to surface were noted. A welder was on location cutting off the top of the casing and installing an insert for the drilling rig to nipple up on. No violations were noted at time of inspection.
2839405 Primary Facility 2019-02-13 Routine/Complete Inspection No Violations Noted Weather was 48° and sunny at time of inspection. Met with Seneca representative Jason Bence. Well is a spud conductor. Well was covered with a top hole rig at time of inspection. Top ho,e rig was drilling the 52H. 0% combustible gas was detected anywhere around the well at time of inspection. No violations were noted at time of inspection.
2844710 Primary Facility 2019-02-26 Routine/Complete Inspection No Violations Noted Weather was 24 degrees and sunny at time of inspection. Met with Seneca representative Jason Bentz on location. Drilling has just started the 17.5' hole for the surface casing. Estimated TD is 620'. No violations noted at time of inspection.
