Spoke with Lenny Long (Ziegenfuss) who was finishing up drilling the conductor for the Hensler 7. He had one more 20’ length to go for the 100’. Also onsite was Jeremy Roberts (Inflection) who noted that the 3H was staked out but would not be drilled at that time because there was no permit yet. Crew were moving some trailers and containers onsite. Inflection hopes to move in the rig in early April. There were no violations at the time of my inspection. Left location @ 12:30.
2579463 Primary Facility 2017-04-05 Routine/Complete Inspection No Violations Noted Routine Inspection; 4/5/2017.

Rebecca Renner (DEP), Sean VanFleet (DEP), and I met with Donnie Stevenson, Greg Malekos, Gregg Saunders, and Jason DuPont all representing Inflection Energy at 08:55. We were invited to the site to look at the containment before drilling equipment is moved onto the site. The pad surface was graded and compacted toward the cellar then 10 oz. Geo. fabric was installed over the modified limestone material. The containment liner was then installed followed by a layer of thick rubber mats (see photos). The top of the containment berm is 9 inches higher than the elevation at the edge of the cellar. Overall, the containment looked good. Surface casing was being delivered and unloaded at the time of the inspection while conductor was already set on the Hensler 7 (081-21660). The site was well vegetated but would not be considered permanently stabilized. PCSM (Post Construction Stormwater Management) controls have been implemented. The fresh water impoundment currently contains

2584721 Primary Facility 2017-04-18 Drilling/Alteration No Violations Noted Arrived @ 10:10. Weather: Sunny, Orion 50 onsite and prepping for drilling. Met with Greg Malakos. The rig was fitted with an oversized top drive to handle the larger casing plan. Walked the compact site, which was well organized. There were no violations at the time of my inspection. Left location @ 11:15.
2586332 Primary Facility 2017-04-24 Drilling/Alteration No Violations Noted Arrived @ 12:24. Weather: Overcast. Orion 50 onsite. Met with Greg Malakos. At the time of my inspection 460’ of 18-5/8” surface casing had been introduced into the well bore, but the casing would not go further down. Drilling the 22” surface casing hole went well, according to Mr. Malakos. A ¼” stream of water was encountered at ¿ 132’ and stopped at ¿ 350’. A deviation survey spaced at ¿ 200’ intervals showed some minor dog legs 1° to 2°, with more deviation at the bottom of the hole. It was decided to use two 22” side reamers to attempt to open the hole enough to take the casing. Preparations were being made to begin this work when I left at 14:42.
2588395 Primary Facility 2017-04-25 Complaint Inspection No Violations Noted Complaint inspection; 4/25/2017. I entered the site at 12:10. At the time of the inspection, the Orion 15 drill rig was onsite but being repaired. Earlier today the operator received complaint from a nearby landowner. After talking with the complainant, a request for water supply investigation (CID# 326420) was initiated. Samples were collected earlier today from the complainant’s water supply. The water in the water supply had a milky white appearance.

The area between the well pad and the complainant's water supply is well vegetated and no recent earth disturbance was noted. After entering the site I talked to Greg Malekos (Inflection) and I explained the purpose of my inspection. While discussing the complaint, Mr. Malekos explained that at 19:00-19:30 on 4/24/2017 (when the turbidity was first noticed in the complainant’swater supply) the Orion rig on the Hensler was reaming the surface hole on the Hensler 7 (081-21660). The total depth on the surface hole of the Hensler 7 was reached several

2588479 Primary Facility 2017-04-25 Drilling/Alteration No Violations Noted Arrived @ 14:50. Weather: Overcast. Orion 15 onsite. Met with Inflection Drilling Superintendent Greg Malakos, Company Man Jerry Roberts, and Drilling Engineer Habib Guerro. At 6:45 this morning, I received a call from Mr. Malakos informing me that Inflection had received a water supply complaint from a neighboring resident. The complaint was referred to WQS Ben Bahner. Overnight Inflection had been using two 22”-diameter side reamers in an attempt to improve the well bore diameter so that the 18-5/8” surface casing could be installed. At the time of my inspection Inflection was trying to get past a tight spot at 206’ KB (KB height = 20.5’). Note: previous IRs refer to the Orion 50 rig. This was a mistake. The rig onsite was the Orion 15. When I left the location @ 16:45 Inflection was still trying to move past the tight spot.
2588569 Primary Facility 2017-04-26 Drilling/Alteration No Violations Noted Arrived @ 08:15. Weather: cloudy, warm. Orion 15 onsite. Met with Inflection Drilling Superintendent Greg Malakos, Company Men Jerry Roberts (day) and Justin Foster (night), and Drilling Engineer Habib Guerro. Asked Mr. Malakos whether any methane was encountered during surface hole drilling. He confirmed that gas measurements were run, but gas was not detected. The well bore program had changed from a 4-string to a 3-string design with 13-3/8” surface casing. At the time of my inspection, Inflection was preparing to install the last two surface casing joints. Operations were waiting on a swege. Surface casing was landed at 733.5’. Halliburton cement crew arrived @ 10:00, followed by OGI Andy Becher. Ran Inflection’s sweep and LCM pill, rapidly established circulation. Job called for pumping 375 bbls cement (1700 sks, 1.24 yd). Good surface returns were obtained (125 bbls). Left @ 16:45.
2588591 Primary Facility 2017-04-27 Drilling/Alteration No Violations Noted Arrived @ 06:45. Weather: warm. Orion 15 onsite. Met with Company Man Justin Foster (night) and . When I arrived the surface cement at the bottom of the holewas being drilled out. Left @ 7:25. Returned at 12:15, met with Company Man Jerry Roberts. Collars were being added to the bottom hole assembly to prepare for directional drilling of the intermediate well bore. Left location @ 14:20.
2590194 Primary Facility 2017-05-02 Drilling/Alteration No Violations Noted Arrived @ 08:30. Weather: cloudy, windy, cool, following heavy storms/high winds which brought down trees. Orion 15 onsite. Met with Company Man Jerry Roberts. Shock sub and MWD had to be replaced. Metal, probably from shock sub, came back with cuttings. Weatherford was running caliper and image logging from from ¿ 734’ to ¿ 2438’ when I arrived onsite. Caliper indicated some wash-outs. Left @ 11:15. Returned at 15:00 met with drilling consultant Carey Carr and drilling superintendent Greg Malekos who were preparing for a bit run. Left location @ 15:30.
2590853 Primary Facility 2017-05-03 Drilling/Alteration No Violations Noted Arrived @ 08:30. Weather: cloudy, overcast, tempertures in 50s. Orion 15 onsite was finishing running intermediate casing with CRT when I arrived. Met with Company Man Jerry Roberts, and drilling superintendent Greg Malekos, who described caliper log as indicating 40% washout (requested Weatherford report). Halliburton crew (Matt Bower and Clint Farley) conducted pressure test @ 11:15. Pumped 25 bbl gell spacer with LCM, then 5 bbl fresh water and 189.1 bbl cement with flake. When rapidly added, the flake caused a brief decrease in pump pressure. Pumped 191 bbl displacement. Bumped plug at ¿ 12:35 at ¿ 1418 psi. Trace cement returns to surface. Left location @ 13:15.
2592315 Primary Facility 2017-05-08 Drilling/Alteration No Violations Noted Arrived @ 08:30. Weather: sun and clouds. Orion 15 onsite, not drilling. Met drilling superintendent Greg Malekos. Had drilled to 5200’ on air, but appeared to be stuck. At the time of my inspection crew was switching from air to SOBM. BOPs were tested on 5/4/17. Left @ 14:25
2592327 Primary Facility 2017-05-09 Drilling/Alteration No Violations Noted Arrived @ 07:39. Weather: cold and clear. Orion 15 onsite, not drilling. KLX wireline onsite. Met drilling superintendent Greg Malekos. Conducted torque test to estimate freepoint at 4640’, nearest collar at 4950. Deepest drilling depth 5257’. Perforated the drill pipe (4902’ – 4905’, 5 shots@ 07:20. Tried to circulate allowing pressure to rise to 2000 psi – pack off was the pressure limit. Did not establish circulation. Looked at graph for FIT test conducted on 5/5/17. Tested to 18ppm mud equivalent. Left @ 8:30. Came back at 13:00 with Oil and Gas Operations Chief John Ryder. Met with drilling engineer Habib Guerra, Greg Malekos, and Jerry Roberts. At the time of my inspection, Inflection had just sent down a ball to try to stop the thief zone causing loss of circulation. This had not worked. Left location @ 13:30.
2594702 Primary Facility 2017-05-11 Routine/Complete Inspection No Violations Noted Spill Response Inspection; 5/11/2017. I entered the site at 12:55. At the time of the inspection, the Orion 15 drill rig was onsite drilling the production hole on the Hensler 7 (081-21660). Gregg Saunders (Inflection) called to report approximately 1 barrel of synthetic oil-based drill mud was spilled above the drill floor from the mud line on the Top Drive. Drill mud was sprayed on the rig and equipment on the south side. I found no evidence that drill mud left the containment and the volume estimate appears to be accurate. No Violations Noted.

2647486 Primary Facility 2017-10-13 Routine/Complete Inspection No Violations Noted I met Justin Foster when I arrived on-site. Orion rig #15 on location drilling the 7A well, at 2806'. Containments were in good condition, with some water built up in the mud tank containment and the chemical containment; Justin said he would see that these were addressed.
2650151 Primary Facility 2017-10-19 Drilling/Alteration No Violations Noted Arrived at 14:50 Weather, sunny and warm. Met with Habib Guerrero, drilling director, Greg Malekos, drilling superintendent, and Justin Foster, company man. Orion 15 on site. At the time of my inspection there was SOBM in the well bore (TD 4340'), and they were moving switchto a PDC bit. Previously drilling on air. Reviewed first IC cement log: pumped 30% excess but there were minimal returns to surface. Floats did not hold. Held pressure on cement head for 4 hours. Cement top at approx. 157', based on a tag from the top. No violations were observed at the time of my inspection
2652517 Primary Facility 2017-10-25 Drilling/Alteration No Violations Noted Arrived at 10:30. Weather, sunny and warm. Conducted inspection with OGI Ulyana Trynovich. Met with Jerry Roberts, company man. Orion 15 was at 6213'. Progress slow, sliding and fighting to maintain wellbore orientation. Discussed lack of mats to protect containment near back corner of pad. No violations were observed at the time of our inspection.
2655750 Primary Facility 2017-10-31 Drilling/Alteration No Violations Noted Arrived at 14:26. Weather: cloudy and cold. Onsite were: Greg Malekos, Jerry Roberts and Jeff Whitehair. Halliburton Sperry onsite conducting managed pressure drilling. Baker onsite operating rotary directional. Hole depth was 6933' at the time of my inspection. Drilling pilot to 9842'. No violations were observed at the time of our inspection.
2658950 Primary Facility 2017-11-09 Drilling/Alteration No Violations Noted Arrived at 10:30 Weather: misty with light rain. Hensler 3H is conductor only. Onsite were: Justin Fowler and Carey Carr (Inflection). Orion 15 is on the skid well for the 7H. Halliburton Sperry onsite conducting managed pressure drilling. Baker onsite operating rotary directional. Pilot hole, plug back and kick off went well. Hole depth was 9258' at the time of my inspection. This was approximately half way towards building the curve. No violations were observed at the time of our inspection.
2663271 Primary Facility 2017-11-21 Drilling/Alteration No Violations Noted Arrived at 13:00. Weather: sunny, with temps in low 50s. Met with Company Man Jerry Rice. Orion 15 finished drilling the skid well for the 7H, and had moved to the 3H. At the time of my inspection, rig was preparing for a reamer run on the surface well bore (20"). Cellars were water filled for the 7H (plugged) and the 7A, Inflection's name for the finished skid well. No violations were observed at the time of my inspection.
2671186 Primary Facility 2017-12-14 Drilling/Alteration No Violations Noted Arrived at 13:00. Weather: sunny, cold. Met with Company Man Justin Foster. Orion 15 on Hensler 3H, tripping in the hole and encoutering tight spots at 5653'. Last week ,severe washouts and steep dips at 4089'. Plugged back 600' to set cement plug approx. 100' up in first intermediate casing (13-3/8": 2810'). Sidetracked off about 100' below shoe at dogleg. Switched to oil-base prior to tripping in. Hensler 7H is plugged. Cellar filled with ice, no bubbling. 7H skid completed. Cellar iced over. No bubblung observed. No violations were observed at the time of my inspection.
2678497 Primary Facility 2018-01-03 Drilling/Alteration No Violations Noted Arrived at 16:00. Weather: sunny, calm, temps in high 20's. Met with Company Man Jerry Price. Orion 15 on Hensler 3H, at 6430'. Got stuck while reaming at approximately 6200' . Were stuck for a week over Christmas. Got unstuck by sending coiled tubing down drill pipe and trying to perf collars. Set second IC at 6255'. Large cuttings were coming from hole at the time of my inspection. Hensler 7H is plugged. Cellar filled with ice, no bubbling. 7H skid completed. Cellar iced over. No bubbling observed. No violations were observed at the time of my inspection.
2681910 Primary Facility 2018-01-11 Drilling/Alteration No Violations Noted Arrived at 14:00. Met with Company Man Jerry Price. Orion 15 on Hensler 3H, at 11869, running to 15005. Effective mud weight is 17 ppg, flare of approx. 1'.

Hensler 7H is plugged. Warm temps had partly melted the ice - no bubbling observed. 7H skid: completed. Cellar bubbling, source not determined. Returned to location on 1/12/18. Cellar was pumped down. Bubbling was from the 9-5/8" x 5-1/2", which was open to vent. No violations were observed at the time of my inspection.

2682512 Primary Facility 2018-01-03 Routine/Complete Inspection No Violations Noted The Orion 15 was onsite drilling the Hensler 3 well (081-21667). Air packs just came off the site but the liner remains onsite. Site containment is in good condition. The backyard and cuttings management look good especially considering cold conditions. No violations noted.
2689904 Primary Facility 2018-01-29 Routine/Complete Inspection No Violations Noted Arrived at 11:00. Weather: sunny, cold. Orion 15 has been laid down. Rig mats in place. Mud tanks being removed. Oil base on mats in many locations. Met with Mike Woodruff. Plans for DFIT in a few weeks. Drilling finished on Hensler 3H.

Hensler 7H is plugged. Hensler7H skid is drilled. Discussed condition of mats snd pad with Greg Malekos who said Inflection plans to remove mats and linear and evaluating the situation before fraccing the wells. No violations were observed at the time of my inspection.

2707879 Primary Facility 2018-03-15 Routine/Complete Inspection No Violations Noted Routine Inspection; 3/15/2018.

At the time of the inspection I was alone onsite and two wells were onsite. Both the Hensler 3(081-21667) and Hensler 7 (081-21660) wells were onsite as well as the original Hensler 7 which was plugged but remains in a cellar. Overall, the site is cleaned-up from drilling operations and containment has been removed. No violations noted.

2709457 Primary Facility 2018-03-22 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection on the Hensler pad was conducted March 23, 2018. At the time of inspection, there were 3 wells on the pad; the 7H original hole, the new 7H hole drilled under that permit number after the original hole was plugged prior to reaching TD (plug and skid event), and the 3H. The 7H (plugged) original hole was capped, and had four annular vents. There was no flow or gas observed at any of the vents for this plugged well. The 7H new hole and 3H both were shut in on manual master valve atop wellhead assembly. All annuli had open vent pipes. No obvious issues were observed. Regarding the 7H original plugged hole; I spoke with the operator about permanent requirements for marking a plugged well and discussed that an erected or buried permanent marker may wait til after the other wells are completed for wellbore access and monitoring purposes during completion.
2790734 Primary Facility 2018-10-16 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of the Hensler 7 original plugged borehole (081-21660-00-00) was conducted October 17, 2018. The original borehole was plugged prior to reaching anticipated TD and a new hole was drilled under the permit as part of a plug and skid event. At the time of inspection, the original plugged borehole is capped and labeled. The cellar was full of gravel with standing water on top; no bubbling observed. There were 4 annular vents extending above ground level from the cellar. There were no open holes, discharges, visible venting, or any obvious issues observed. The Hensler 7H new hole (081-21660-00-01) surface location is situated right next to the original plugged borehole (see photos).
2819588 Primary Facility 2018-12-27 Routine/Complete Inspection No Violations Noted Two wells drilled and not staged for production. One well plugged and skidded(081-21660).
