2511038 Primary Facility 2016-08-09 Routine/Complete Inspection No Violations Noted I entered the site off Spook Hollow Road at 10:56. The RCE (Rock Construction Entrance) has been installed and the perimeter E&S controls have been installed. At the time of the inspection, the contractor was scraping topsoil off the well pad area and stockpiling it on the southeast corner of the location as specified in the plans. No E&S issues were noted during the inspection and the area downslope of the earth disturbance is well vegetated, further reducing the potential for accelerated erosion. Spook Hollow and Rose Valley Roads were both upgraded prior to commencing earth disturbance on the well site. No Violations Noted.
2520326 Primary Facility 2016-09-14 Preoperation Inspection No Violations Noted I entered the site off Spook Hollow Road at 08:10. At the time of the inspection, the well pad’s limestone surface course was being installed and topsoil was being re-applied to graded slopes. The major earthmoving for the site is complete and most of the previously disturbed soils have been tracked-in and hydro-seeded. The infiltration trench has also been installed on the fill side of the pad as designed. Overall, the site is nearly complete. E&S controls remain installed and appear to be functioning properly. This inspection fulfills the Pre-Operational Inspection requirement of the O&G Act as amended by Act 13. No violations noted.
2550875 Primary Facility 2017-01-03 Routine/Complete Inspection No Violations Noted Routine Inspection; 1/3/2017. I entered the site off Spook Hollow Road at 09:54. At the time of the inspection, the well pad was completed and two well cellars were installed. Since the last inspection, a flash-flooding event on 10/20-21/2016 caused some damage but repairs were accomplished immediately afterward. Due to saturated conditions, relatively young vegetation, and freeze/thaw the site has been experiencing erosion since the growing season has ended. Currently, several layers of E&S controls have been installed and it appears the E&S issues have been confined to the LOD. Sediment is, however, accumulated along the uppermost E&S controls and significant erosion exists along the western edge of the well pad. I recommend evaluating ways to further minimize erosion until conditions become more favorable for a permanent solution. No Violations Noted.
2570509 Primary Facility 2017-03-10 Routine/Complete Inspection No Violations Noted Routine Inspection; 3/9/2017.

I entered the site off Spook Hollow Road and met with Kirk Gregory (Seneca) at 12:05. We met to discuss the ongoing erosion along the west and southwest side of the pad which began during an extreme rainfall event in the fall of 2016. The erosion has continued to get worse but has not impacted areas outside the LOD because several layers of silt sock have been added downslope of the pad. Seneca plans to armor the channel that is eroding (photos) and implement the PCSM plan when conditions improve. We also discussed routing some of the water from the upslope diversion in a different direction. Since the Department already has a major modification for the ESCGP, I suggested talking to Daniel Harvey (DEP) for any additional changes. We discussed how this one drainage pathway caries water from a majority of the well pad. It appears that this concentrated flow is responsible for scouring the west/southwest side of the well pad. 2591336 Primary Facility 2017-05-05 Routine/Partial Inspection No Violations Noted Routine Inspection; 5/5/2017. I entered the site at 11:20 to monitor the effectiveness of the E&S controls installed around the perimeter of the site. Erosion caused by significant rainfall in the fall of 2016 has been an ongoing concern on this site. Since the last inspection, the contractor has started to install rock armor along the ditch conveying water from the cut slope diversion. Erosion was visible but perimeter E&S controls were able to settle the sediment from the water before it left the site. Please continue to maintain controls as needed. No Violations Noted.
2606277 Primary Facility 2017-06-15 Routine/Partial Inspection No Violations Noted Routine Inspection; 6/15/2017. I entered the site at 13:19 to inspect installation of the PCSM features and the upgrade of the drainage system. Perimeter E&S controls are maintained and appear to be effective. Vegetation on the site outside the recent earth disturbance is well established and looks good. The bio-retention berm is being constructed but in not complete. Please be mindful of compaction in the bio-retention berm as compaction is evident. The production lines from the well cellars to the proposed location of the production equipment are installed and the pad surface has been re-graded around the well cellars for containment. Overall, the site is in good condition. I recommend seeding and stabilizing the dike/berm portion of the bio-retention berm when that part is complete. No Violations Noted.
2646043 Primary Facility 2017-10-11 Routine/Complete Inspection No Violations Noted Arrived at 13:40. Staff onsite working on large sight wall. Two conductors and cellars built. There were no violations observed.
2731710 Primary Facility 2018-05-18 Routine/Complete Inspection No Violations Noted Weather was 60° and sunny. Met with Seneca representative Gus Trejo on location at time of inspection. Well is a spud conductor. Well was covered by a rig mat at time of inspection. No combustible gas was detected at time of inspection. No violations were noted at time of inspection.
2731741 Primary Facility 2018-05-21 Routine/Complete Inspection No Violations Noted Weather was 52° and cloudy at time of inspection. Met with Seneca representative Gus Trejo on location for inspection. Rig was in the process of drilling the 12 1/4' hole for the intermediate casing. They are estimating to drill that hole to around 3000' ft. Reviewed the cement job log from the surface casing and no discrepancies were noted. Conducted a walk around inspection of location. No violations were noted at time of inspection.
2735408 Primary Facility 2018-05-30 Routine/Complete Inspection No Violations Noted Weather was 75° and cloudy at time of inspection. Met with Seneca representative Kevin Bradford on location at time of inspection. Rig was in the process of drilling the hole for the 8 3/4" hole for the production casing. They estimate completing drilling on 05/31/2018. Reviewed the intermediate casing cement job and no discrepancies were noted. Conducted a walk around inspection of location. No violations noted at time of inspection.
2765763 Primary Facility 2018-08-21 Routine/Complete Inspection No Violations Noted Weather was 66° and raining at time of inspection. There were no Seneca resources representative on location at time of inspection. Wells have recently been stimulated and are in production. Well had digital pressure monitoring equipment attached to the tubing and to the production casing. Tubing pressure was 1468 and casing pressure was 2232 at time of inspection. Due to the inclement weather I did not test the annulus vents with Altair 5x gas meter. Will follow up for gas metering in the next couple days. No violations noted at time of inspection.

There were no Seneca resources representative on location at time of inspection. Wells have recently been stimulated and are in production. Well had digital pressure monitoring equipment attached to the tubing and to the production casing. Tubing pressure was 1668 and casing pressure was 2362 at time of inspection. The well had 3 annulus vents plumbed to surface at time of inspection. 0% combustible gas was detected on any of the annulus vents at time of inspection. No violations noted at time of inspection.
