2410616 Primary Facility 2015-09-24 Routine/Complete Inspection No Violations Noted I arrived onsite with Dan Harvey (DEP) at 09:30. We immediately met with Jason DuPont (Inflection). The RCE (Rock Construction Entrance) and perimeter E&S controls were both installed and functioning properly. The infiltration basin was being constructed with spoils from the FWI (Fresh Water Impoundment), which was also being constructed. Topsoil was stripped from the active construction and the access road going up to the well pad. Stockpiled soil was seeded and stabilized. The well pad remains in pre-construction conditions and has not been disturbed. Also, rock has been applied on the access road where the NFG (National Fuel Gas) pipeline crosses. Overall, the construction and sequence look good. No Violations Noted.
2464847 Primary Facility 2016-03-15 Preoperation Inspection No Violations Noted Preoperational Inspection; 3/15/2016.

At the time of the inspection, the entire site was constructed including the well pad, access road, and FWI (Fresh Water Impoundment). PCSM (Post-Construction Stormwater Management) features including infiltration berms and the infiltration basins are stabilized and appear to be functioning properly. Perimeter E%S controls were installed and functioning properly. Overall, the site is in excellent condition. This inspection fulfills the pre-operational inspection requirement of the O&G Act as amended by Act 13. No Violations Noted.
