Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2317318 Primary Facility 2014-10-29 Routine/Complete Inspection No Violations Noted Pre-Operational Inspection with Sean Van Fleet (DEP); 10/29/2014. At the time of the inspection the perimeter E&S controls were installed, the infiltration basin/berms were installed and hydro-seeded, and the topsoil was being stripped and stockpiled upslope of the proposed pad location. Due to the quality of the existing access road, the entrance to the well pad has not yet been upgraded with exception to the addition of a RCE (rock construction entrance). Overall, the pad is in good condition and existing stream crossings remain at their original elevations. This inspection fulfills the pre-operational inspection requirement of the O&G Act as amended by Act 13. No Violations Noted.
2340329 Primary Facility 2015-01-30 Routine/Complete Inspection No Violations Noted Arrived @ 9:47 on 1/30/15, weather cold with snow squalls, pad snow-covered. There are 4 cellars in a linear array on the pad. There was conductor in the outermost two (cellar nearest the pad entrance and furthest away). The other two cellars were covered at the time of my inspection. There were also some materials visible on the pad including: a collection of jersey barriers and a dumpster. Left location @ 10:10.
2545697 Primary Facility 2016-12-15 Routine/Complete Inspection No Violations Noted Routine Complete Inspection; 12/15/2016.

At the time of the inspection the site was covered in snow and temperatures were below freezing. Four cellars had only conductor set and capped for the Reitz 1 (081-21520), Reitz 2 (081-21500), Reitz 3 (081-21527, and Reitz 6 (081-21521). Conductor was set on the site back in January of 2014 without any further activities on the wells. The site appears to be constructed to the PCSM phase specifications. I recommend drilling with due diligence and implementing the site restoration plan or applying for a 2-year extension and terminating the ESCGP when vegetation is adequate. No Violations Noted.
