I entered the location at 12:18 with Jared Rutt (DEP) and began the inspection. At the time of the inspection, Halliburton was finishing the hydraulic frac on the 15th stage on the Kenmar A 1H (081-21454). We walked the site with the company man and noticed some water in containment that had water between 3,000-4,000 umhos/cm. No indication was found of any leaks around the produced water tanks or around the secondary containments under the blenders or individual equipment. I recommended vacuuming the water off the containment when possible. Overall, the site is in good condition. This is the only well scheduled for completions at this time.

The well pad area and access road are all stabilized and no erosion was noted. The well site remains at its originally constructed size and the operator plans on applying for an ESCGP-2 to accommodate Post Construction Stormwater Management (PCSM) according to the identified in the restoration assessment submitted on August of 20

2697852 Primary Facility 2018-02-13 Routine/Complete Inspection No Violations Noted I entered the site and spoke to the company man (Keith Heiken) and we walked the site. Conditions were sunny and above freezing with snow cover. The rig was snubbing tubing on the Kenmar 2H at the time of the inspection. The containment berm on the south/southeast side was worn and less effective than other parts of the berm. The company man said they would be moving the workover/snubbing unit off location within about a day. I recommended monitoring the containment berm since they were nearly complete. The company man pointed out the vac truck onsite and said they have been vacuuming the containment as more snow and ice melts. No violations noted.
2752236 Primary Facility 2018-07-06 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection was conducted July 6, 2018 on 3 conductors for which a Notice of Intent to Plug was submitted. Plugging was scheduled for earlier this week. At the time of inspection, all 3 conductors were below grade with plates over them. Mouseholes were also covered with plate. Cellars were intact with grading across the top. Plugging could not be verified by this inspection. No obvious issues were observed at the three conductors at the time of this inspection.
