It was obvious a second round of hydro-seed was applied to areas of sparse growth. Roadside ditches and the road surface itself were in excellent condition. Standing water was noted in both basins. I recommend monitoring these basins outside the growing season and acting if needed to promote effectiveness during the next growing season. The pad area has some vegetative growth beginning and the entire area has been hydro-seeded.

2381541 Primary Facility 2015-06-19 Incident- Response to Accident or Event No Violations Noted The release was caused when heavy rain washed brine off containment liner which had the berms removed to allow equipment to be removed from the site. At 13:35, I arrived onsite and met with Mr. Subra. Much of the pad previously impacted by a release on 6/9/2015 was excavated to allow for the installation of the flow lines from the well heads to the production units and produced water tanks. Since the last inspection, only a small amount of containment remains around the well heads and the adjacent flowback tanks. The remaining containment was cut-back toward the west where the blender and sand equipment was located. It was from this smaller cut-back containment that the most recent release originated. An intercept trench was installed along the west side of the remaining containment and conductivity ranged from 2,500-2,700 umhos/cm while a nearby puddle was 5,250 umhos/cm. Puddles south of this containment were approximately 650 umhos/cm and water ponded around the well heads and flow lines ranged from 1,200-1675 umhos/cm.
