Hamilton Unit 3H Top gauge: 1090.58 psi (digital) 5-1/2” gauge: 1500 psi (0-5000 psi, analog) 20” x 13-3/8”: 17% methane 13-3/8” x 9-5/8”: 0 % methane 9-5/8” x 5-1/2”:100% methane, discolored fluid dripping out of annulus. Discolored gravel below annular vent.

2465151 Primary Facility 2016-02-18 Follow-up Inspection No Violations Noted Arrived @ 10:33. Weather: sunny and cold. With me was Ken Halk (Penn E&R) to jointly monitor annular vents. We met on location to investigate discrepencies between my annular vent monitoring data and MIA data recently collected by Ken on behalf of Inflection. Inflection’s MIA protocol called for monitoring annular vent flow rates, but did not call for monitoring methane concentrations within the annular vent. DEP utilizes detection of methane within an annular vent that is cemented to surface as an indication of defective cement. Ken’s monitoring data and my own were compatible when we monitored at the same location. I monitored the annular vents for methane gas with an Altair 5X gas meter calibrated on 2/17/16.

2476493 Primary Facility 2016-04-06 Follow-up Inspection Outstanding Violations - No Viols Req'd Arrived @ 10:00. Weather: cold and clear. The purpose of this inspection was to follow-up on previous inspections in which field monitoring detected methane outside of surface casing (on the 20” x 13-3/8” annulus).

Hamilton Unit 3H, well in production Tubing pressure 1450 psi (digital) 5-1/2” casing pressure: 1500 psi (digital) 20” x 13-3/8”: 7% methane 13-3/8” x 9-5/8”: 0 % methane 9-5/8” x 5-1/2”: annular vent plumbed to polytank, 5% methane at tank outlet.

2522890 Primary Facility 2016-09-27 Routine/Complete Inspection No Violations Noted I entered the site off Heilman Road at 09:37. The GP-11 crossing of the UNT to Mill Creek is well vegetated and remains in good condition. I continued up Spike Lane to access road. The fresh water impoundment has several feet of freeboard and the area is well vegetated. Three wells (081-21240, 081-21465, and 081-21467) on the pad were in production at the time of the inspection. Temporary sand separators remain installed and plumbed into a gas buster with 6 additional frac tanks in containment. The site is well vegetated throughout although the infiltration basin had standing water up to the orifice but was not discharging (although it did rain less than 12 hours ago). Approximately 1/3 of the basin is vegetated with cattails. I recommend evaluating the infiltration capacity of the basin. No Violations Noted.

The linear array of wells in the field is as follows: HU 1 6 3 no label 081-21240 081-21467 no permit number filled cellar

The array of wells observed in the field appears to be at odds with the permitted well locations. Based on the permits, the Hamilton Unit 1 well should be located approximately 30’ north of the Hamilton 6H. From west to east the 6H, 3H and 5H should be located in a roughly west-east array.

Sent message to Tom Gillespie requesting an explanation for the apparent locational anomalies.

There were no field violations observed at the time of my inspection. Left @ 11:20.

2596559 Primary Facility 2017-05-16 Routine/Complete Inspection No Violations Noted Routine Inspection; 5/16/2017. I entered the site off Heilman Road at 12:50 to evaluate the fresh water dewatering plan submitted on 5/8/2017. The primary dewatering location is the topsoil stockpile south of the fresh water impoundment and the secondary dewatering location is the fill slope to the east of the impoundment. Both dewatering areas are well vegetated and can be considered permanently stabilized. I recommend monitoring to ensure that the discharge rate does not exceed the infiltration rate. Overall, the dewatering plan describes how the operator will maintain compliance during the discharge of fresh water. The Department acknowledges receipt of the Freshwater Impoundment Dewatering Plan for the Hamilton Well Site. No Violations Noted.
2605661 Primary Facility 2017-06-20 Administrative/File Review Violation(s) Noted Administrative Inspection entered for the purpose of entering an Enforcement Record (NOV).

Inflection has acknowledged that the Hamilton 3H well was drilled to a deeper TVD than the anticipated maximum TVD on the well permit due to geologic uncertainty. Based on Inflection’s acknowledgement of exceeding the anticipated maximum TVD, the Department has determined that re-permitting this well is not required. Thus, violation cited in Inspection ID 2605661 has been updated to “Administratively Closed Out” as of 9/18/2017.

2629821 Primary Facility 2017-08-24 Complaint Inspection No Violations Noted I met onsite with the complainant about water being discharged from the fresh water impoundment onsite. The complainants showed me where water was encountering the driveway. This saturated area was then rutted somewhat by the traffic on the driveway. A cross culvert on the driveway was also pointed out. I recommended the operator relocate the discharge or re-evaluate the discharge rate.

On 8/25/2017, the operator contacted me and said that today would be the last day of discharging water in 2017. The operator explained that once the current fuel tank was empty(3 hours), the discharge would be finished. No violations noted.

2637728 Primary Facility 2017-09-19 Routine/Complete Inspection No Violations Noted Three producing sells on location:1H, 6H, & 3H. 5H is conductor only.3H no gas on any annuli (Altair 5X gas meter calibrated on 9/19/17. Result of this inspection is No Violations.
2661164 Primary Facility 2017-11-14 Routine/Complete Inspection Outstanding Violations - Viols Req'd Arrived at 9:43. Weather: cloudy, temperatures in the 40's. No personnel were on site. There were a total of 4 wells: 3 producing wells: 1H, 6H, and 3H. The 5H was not TD'ed. These wells are in a linear array. There appears to be a minor brine leak at one of the 1H sand separator valves (see picture). Each producing well has 3 open annuli which were monitored for combustible gas. The 6H 9-5/8" x 5-1/2" annulus yielded 100% methane/ volume (Altair 5X gas meter calibrated on Nov 9). No new violations were observed during this inspection.

2840525 Primary Facility 2019-02-14 Routine/Complete Inspection No Violations Noted No activity on the site. The 1H, 3, and 6 wells were in production, with a sign marking the location of the undrilled 5 well. The production tank containment was full to approximately 6" of the top; there is certainly less than 110% of the volume of one of the production tanks. I recommend that this containment be cleared as soon as possible. The GPU containment was also full of ice and snow to within 1-2" of the top, and the metal berm was obviously bowed on the right side of the containment. I recommend that this also be cleared and the metal berm assessed and repaired if necessary.
