2410509 Primary Facility 2015-09-21 Routine/Complete Inspection No Violations Noted I arrived onsite at 16:15 after talking to Becky Renner (DEP) about some bubbling in the infiltration basin. We walked down to the infiltration basin and talked with Thomas Gillespie (Inflection) and Jason DuPont (Inflection consultant). Bubbles were visible in the lowest part of the infiltration basin and it was clear that the basin was not infiltrating even with amended soils. After evaluating the mucky bottom of the basin and walking across the basin it was noticed that the bubbles were especially concentrated where my footsteps had disturbed. I attempted to confine and sample the gas, but the volume was insufficient. I was able to attach the MSA Altair to a modified 500ml bottle to get methane at 30% LEL (max). 2520332 Primary Facility 2016-09-14 Routine/Complete Inspection No Violations Noted I entered the site off Yeagle Road at 08:34 and met with the Company Man (Frank Covine). At the time of the inspection, the CalFrac Well Services was onsite actively completing (perforate and hydraulic fracture) the TLC 2 (081-21470). Mechanical breakdown on the crane had temporarily shut down the wireline and active operations. Three of the four wells on the site are scheduled to be completed at this time. Containment looks good and well maintained throughout the site. The well pad surface and access road appear to be holding up well to the rather intense truck traffic. The road had been wetted and I saw a road sweeper onsite. Overall, the site looks good. No Violations Noted.
2523542 Primary Facility 2016-09-22 Incident- Response to Accident or Event No Violations Noted Routine Inspection; 9/22/2016.

Inflection Energy notified the Department of a release of produced water/blended water to the gravel pad surface. According to the notification, the elevated conductivity was just recently discovered and the release was estimated at 25 gallons but the release was being investigated further. I entered the site off Yeagle Road at 14:28 and met with Inflection Energy representatives. At the time of the inspection CalFrac was completely rigged-down and had left the site. Onsite at the time were the two fresh water tanks, one waste tank, centrifuges, filters, working tanks, sand equipment and containment around the well heads. Personnel onsite were vacuuming water which was accumulated on the gravel pad surface between the working tanks and the sand equipment (north of the well heads). Personnel onsite also excavated pad material on the north and east sides of the working tanks. Water in both of these excavations (on the pad surface) had field conductivity between 800-1,100

2537470 Primary Facility 2016-11-16 Routine/Complete Inspection No Violations Noted Complaint Response Inspection; 11/16/2016.

Today the Department received a complaint (CID# 323627) about noise from the TLC well pad. I met onsite with the Company Man (Frank Covine) and he explained the use of a vac-truck to remove accumulated sand from the tanks onsite today. Currently, they drilled plugs out on the TLC 1 (081-21461) and are waiting for a snubbing unit to begin drilling out plugs on the other two wells. Mr. Covine said they waited until after 08:00 this morning to begin using the vac-truck due to the noise associated with this activity. The complainant, however, was concerned about the noise over the past few months and not the specific noise associated with today’s activities. In fact, the complainant made it clear the noise today was quite a bit less noticeable than on previous days when hydraulic fracturing was active and the well was under pressure. 2540098 Primary Facility 2016-11-28 Routine/Complete Inspection No Violations Noted Arrived @ 14:00, weather: sunny, warm. TLC 1 was flaring at the time of my inspection. The flare stack was set up near the pad entrance and the flare appeared to be approximately 40’-to 50’ high. Began flowing back at approximately 7 am this morning, unloaded water quickly. Called Inflection Drilling Superintendent Greg Malekos to ask about the flare. He said that Inflection thought the well would produce more water and not be ready to produce gas for some 3 to 4 days after flowback began. At the time of my inspection, the Energy Transfer Partners pipeline was not ready, but the gas would be flowed to production as soon as possible. Left location @ 14:45.
2541335 Primary Facility 2016-11-30 Complaint Inspection No Violations Noted Complaint Inspection; 11/30/2016. I received concerns from nearby property owners on 11/29/2016 concerning flaring during the flowback. I understand that the pipeline became available at approximately 19:00 on 11/29/2016 and the TLC 1 was put into production. While driving by I noticed ongoing flaring at the site but clearly the volume of gas being flared was low and substantially less than on the previous day. The TLC 1 well is hooked up to pipeline and is producing while also flaring gas. I talked to Greg Malekos from Inflection Energy and mentioned I would discuss the situation with DEP’s Air Quality Program. Later on 11/30/2016, Air Quality responded to Inflection and provided the guidelines for completions when pipeline is present. No Violations Noted.

2566273 Primary Facility 2017-03-01 Routine/Complete Inspection No Violations Noted I entered the site off Yeagle Road at 14:26. At the time of the inspection, three wells onsite were in production (081-21461, 081-21470, 081-21472). The TLC 2 and TLC 4 were on temporary separators and heaters (connected to a flare stack) while the TLC 1 was producing through permanent GPU’s. All of the temporary production equipment, sand separators, flare stack, and high pressure iron are not on containment. Although these items are not required to be in containment, I recommend containment around temporary production equipment and iron. The wells were flowed-back into the above-ground waste tank. Currently, a second waste tank has been installed onsite and the water in the first tank will be treated and moved into the second waste tank. The second waste tank was approved with an OG-71 which was approved on 1/31/2017. The treatment of the flowback onsite began today. Before inspecting the site, I parked near the complainant’s residence (turned off the truck) and listened. I could hear a relatively faint noise in the background. I entered the site off Yeagle Road at approximately 14:30 in response to a noise complaint received on 3/6/2017. The same complaint was documented by CID# 323627 on 11/16/2016. The complainant alleges that noise from the site is making sleep difficult. The complainant specifically said the noise during the day was not really a concern. The wells onsite were completed and have been in production for several weeks. When I entered the site I immediately noticed a vac-truck (supersucker) which was making substantial noise removing sand from the gas buster (as noted in my 11/16/2016 inspection). I talked with Inflection Energy representatives and we walked the site. At the time of my inspection, a crew was on site cleaning out GPUs and sand separators. There are three wells in production on this location. Also three sand separators, 2 large circular water tanks, and 15 baker tanks onsite. Cellars were gravel-filled for each of the three wells.

2609905 Primary Facility 2017-06-30 Incident- Response to Accident or Event No Violations Noted Spill response inspection; 6/30/2017.

I entered the site at 09:45 and met with Mike Goodman (Inflection) in response to a release of approximately 15-20 gallons of produced water which occurred at approximately 12:30 on 6/29/2017. Inflection Energy properly reported the release yesterday. Mr. Goodman showed me the area where the release was discovered when containment was removed after the snubbing unit was moved off the location. Field conductivity with a soil conductivity meter (adding DI water due to dry conditions) was between 290-300 umhos/cm. I found no evidence of the release. Violations noted and immediately corrected.

2649601 Primary Facility 2017-10-19 Routine/Complete Inspection No Violations Noted Arrived at 8:45. Weather: foggy. No personnel were onsite at the time of my inspection. Onsite were 2 large above ground storage tanks, 16 Baker tanks on containment, 6 GPUs, and 3 producing wells.

TLC1H in production. Digital pressure gauges could not be read TLC 2H in prod. 5-1/2": 1500 psi, analog 0-5000 psi TLC4H, in prod 5-1/2": 1500 psi analog, 0-5000 psi. Left at 9:15. There were no violations observed.

2664840 Primary Facility 2017-11-13 Incident- Response to Accident or Event No Violations Noted Incident Response Inspection 11/13/2017.

At 7:56 am, Donald Stevenson (Inflection Energy) called me to report the release of 1,500 barrels (1 barrel = 42 gallons) of treated produced water from the TLC well site at 5:30 am. The release occurred while the treated produced water was being transferred via a pump from an aboveground storage tank (AST) to a frac tank. The release flowed off the well pad into an Unnamed Tributary (UNT) to Loyalsock Creek north of the well site (hereafter Trib. A). The release had not reached a second UNT to Loyalsock Creek which flows south of the well site (hereafter Trib. B). Tribs. A and B converge into one stem below the TLC well site (hereafter Trib. C).

At 9:00 am, Sean VanFleet (DEP), Clark Smith (DEP), and I went to the location in Trib. C where it crosses Baxter Road. Sample 3440315 was collected from this location, designated Sample Point (SP)-L (field conductivity = 2,000 umhos/cm).

At 9:12 am, Department staff arrived at the TLC well site and met with Inflection

2669300 Primary Facility 2017-11-15 Incident- Response to Accident or Event No Violations Noted Incident Response Inspection 11/15/2017.

10:12- Sean VanFleet (DEP) and I entered the TLC well site at approximately 10:12. The pump-around remained installed and water was being vacuumed from Trib. A at SP-D (immediately downstream of the cross-culvert on the access road; see map). After talking to Inflection Energy representatives, we prepared to collect samples with a Penn E&R (DJ Jackson) and Inflection Energy representative (Landon Vanord). 10:39- We walked upstream and were shown a seep upslope of SP-B. This seep had field conductivity of 6,000 umhos/cm (41.335, -76.9285). 10:48- We went upstream on Trib. A to SP-A, which we believed was background. Field conductivity was 112.6 umhos/cm. Sample 3440320 was collected at SP-A at 10:55. 11:10- Sample 3440321 was collected at 11:10 from location TLC-A which is the sump adjacent to the access road. Field conductivity was 14,470 at 11:12. 11:51- Sample 3440322 was collected from SP-D at 11:42. Field conductivity in the cross culvert was 4,350 umhos/cm

2669410 Primary Facility 2017-11-16 Compliance Evaluation No Violations Noted Compliance Evaluation Inspection. After collecting water samples from nearby water supplies Sean VanFleet (DEP) and I entered the site and talked to Landon VanOrd (Inflection). At the time of the inspection, the site surface area was being delineated using soil conductivity. Impacted gravel from the surface of the site was being scraped and stockpiled on containment. Treated produced water was being removed from the northern Aboveground Storage Tank (AST) so that the tank can be removed. Delineation of the site and excavation of impacted material is ongoing and is planned to continue after the two AST's are removed. The pump-around in Trib. A continues. Water continues to be pumped from Trib. A inside the pump-around for disposal. No violations noted, violations previously cited to well 37-081-21472 remain outstanding.
2669432 Primary Facility 2017-12-01 Compliance Evaluation No Violations Noted Compliance Evaluation Inspection. After entering the site I observed plastic covering the sump designated as TLC-A on the map and the areas immediately upslope. The pump-around remains installed to pump fresh water around the impacted area adjacent to the access road culvert. On the site surface, the two Aboveground Storage Tanks (AST's) were removed and the delineation and excavation of the site was ongoing. I talked to Mike Goodman (Inflection) briefly about the cleanup and progress since the last inspection. Several locations on the site were excavated to bedrock (see photo 6). Excavated material was being stored on containment (and is covered when applicable) while awaiting disposal. A plastic-lined sump was installed between the pad and the east infiltration basin to intercept any water that might seep from the fill slope. Before leaving the site, I met with Gregg Saunders (Inflection) and we discussed the site conditions and overall progress. Cleanup activities continue onsite. No violations note
2676042 Primary Facility 2017-12-20 Compliance Evaluation No Violations Noted Compliance Evaluation Inspection; 12/20/2017.

After entering the site, Sean VanFleet (DEP) and I met with Ken Halk and Matt Tusing (both Penn E & R) on the TLC well site. A substantial amount of material had already been removed from the site and screening/sampling was ongoing. At the time of the inspection, a subcontractor was drilling monitoring wells east of the well pad. Several monitoring wells were visible since the last inspection and concrete bases and risers were being installed. Excavation of impacted soils including the installation of a sump was visible east of the well pad, upslope of the eastern infiltration basin. Pumps were staged to be able to collect water from the sumps during rain events. Water from Trib. A, continues to be pumped around the impacted area and water is being collected from SP-D(downstream of the cross culvert on the access road) for disposal. After arriving onsite, I met with Matt Tusing and Andy Clark (both from Penn E&R). Since the last inspection, additional excavation occurred between the access road and the sump (depicted in photo#4). The pump-around was not being operated but water from the sump was being vacuumed when I entered the site. We split samples from nine (9) locations while Penn E&R collected additional samples in accordance with the established sampling protocols. The samples are listed below and the map/photo section provides additional information and documentation. Violations cited to well 081-21472 on 11/13/2017 remain outstanding, no violations noted.

I entered the TLC well site at approximately 11:15. No one was onsite at the time of the inspection and three wells were in production. The sump along the access road (photo #3) has a submersible pump installed but was not in operation during the inspection. The pump-around is no longer installed. Nine (9) surface water samples were collected as listed in the sample section and depicted on the maps. Violations cited on 11/13/2017 to well permit 081-21472 remain outstanding except 78A.56(a) which was closed as of 3/2/2018.
