2286442 Primary Facility 2014-07-10 Preoperation Inspection No Violations Noted At 10:00, I met onsite with Steve Barondeau (APC) and Chuck Simpson (Hawbaker). The site has been cleared, earthmoving is complete, and perimeter E&S controls have been installed. Infiltration trenches have not been installed but are planned to be installed on the three fill sides of the pad after rock is added to the pad surface. Overall, the site looks good and no erosion was noted. This inspection fulfills the Pre-Operational inspection requirement of the O&G Act as amended by Act 13. No Violations Noted.

I entered the location at 13:30 and began the inspection. At the time of the inspection, the three wells onsite (081-21430, 081-21440, and 081-21482) were covered with sheet metal. Conductor was set in 2014 and it appears no further drilling was ever initiated. The well site was in good condition and no E&S issues were observed. The site has no active well permits that have not been drilled. The PCSM/Restoration plans depict the well pad remaining at its originally constructed size and the installation of infiltration berms. Currently, the infiltration berms have not been installed. No Violations Noted.

All three 20" conductors are topped off with gravel to surface. Cellars are intact, full of water. No plugged well monuments yet. No obvious issues were observed.
