2675701 Primary Facility 2017-12-21 Routine/Complete Inspection No Violations Noted Weather was 30° and sunny at time of inspection. There were no ARD representatives on location at time of inspection. Well has a digital pressure gauge attached above swab valve, on the 5 1/2 x 9 5/8 annulus and on the production casing. Tubing pressure read 1041 psi, casing pressure was 1154 and annulus pressure was 4 psi at time of inspection.
2804449 Primary Facility 2018-11-14 Routine/Complete Inspection No Violations Noted Routine site inspection of Alta's Bonnell Run Pad D well pad on November 14, 2018. There are a total of 4 producing Marcellus wells located on this well pad. Each well is properly labeled. There are (4) 400 bbl produced fluids tanks stationed in secondary containment. A trace of water was observed within this secondary containment. Produced fluids tanks are in good condition. No corrosion was observed on the exterior of the tanks.

The well pad has been partially reduced in size as part of the PCSM plan. Some recently new tree plantings were observed along the N and NW reclaimed area. Overall, the well pad is stable. The Department recommends removing the compost sock along the reclaimed area as it is beginning to deteriorate.

I observed stakes along the well pads access road for PCSM compost infiltration berms. These infiltration berms have not been installed to date.
