The access road has been completed and good vegetative growth has been established. Perimeter E&S controls have been installed and are functioning properly. The northern-most part of the fill slope is being reconstructed and should be complete relatively soon. Overall, no E&S issues were observed. This inspection fulfills the pre-operational inspection requirement of the O&G Act as amended by Act 13. No Violations Noted.

2340519 Primary Facility 2015-01-29 Incident- Response to Accident or Event Violation(s) Noted On 1/28/2015, Range Resources reported a release of diesel-based drilling mud from the Union 209 rig while drilling the 10H well (081-21298). The pressurized release was caused by a failed weld on the flow (mud return) line and caused drill mud to be sprayed approximately 300'. The majority of the spill was on containment but an unknown volume sprayed on nearby mountain laurel, trees, and snow off the well pad. On 1/29/2015, I inspected the site. The Union 209 was idle while repairs were being made to the flow line. The majority of the released diesel-based drilling mud was excavated from the woods off containment but impacted trees remain standing. We marked the furthest extent of the release with pink surveyors tape and the impacted trees with orange surveyors tape. A crew was onsite preparing to remove the remaining impacted material while previously excavated material (impacted) was stockpiled on containment. On 1/30/2015, all impacted vegetation and snow removed for disposal. Violations Cited on 1/29/2015 were resolved on 1/30/2015

After arriving onsite I spoke with the company man (Von Coggins) and we walked the site. The containment was in good condition overall and no holes or tears were noted. Pressure washing has been used recently since the weather has warmed and the backyard/cuttings management looks very good. Some drilling mud was accumulated on the liner where the flow line failed several weeks ago. Overall, the site looks good and the containment has obviously been vacuumed on a consistent basis since the snow and ice have been melting. No Violations Noted.

An area approximately 225’X100’ was impacted by the release but an estimate of volume was not available. 2/18/2016: At approximately 08:48 I arrived onsite and met with Randi Mathew (Range). After shoveling, vacuuming, and excavating snow by several means, the area including the fenced pipeline enclosure back to the produced water tanks (release source) had conductivity elevated above what would be considered fresh water or background. Several grab samples within the release area had conductivity between 3,000-5,000 umhos/cm. Based on the evidence, it appears the excavation of the release based on the visual extent was not effective at removing impacted snow. I recommend characterizing the site and following the Department’s spill guidance.

2451686 Primary Facility 2016-02-08 Administrative/File Review No Violations Noted The Well Site Restoration Reports submitted to the Department for the Laurel Hill 9H (081-21298), Laurel Hill 10H (081-21299), and Laurel Hill Unit 11H (081-21300) contain no waste disposal information and are considered incomplete. I recommend evaluating the PCSM (Post Construction Stormwater Management) requirements and re-submitting new Well Site Restoration Reports in the future. No Violations Noted.

There were no Range resources company representatives on location at time of inspection. Tested 13 3/8 x 20 annulus vent. 0% combustible gas was detected. There was no 9 5/8 x 13 3/8 annulus vent observed. 5 1/2 x 9 5/8 annulus vent was plumbed to a poly tank. Well had a 0 to 5000 PSI gauge attached above the swab valve. Gauge was reading approximately 1200 psi a time of inspection.
2664955 Primary Facility 2017-11-28 Site Restoration No Violations Noted Well site Restoration/Notice of Termination inspection; 11/28/2017. The Site Restoration plans have been implemented and contain the required Post Construction Stormwater Management (PCSM). The site is permanently stabilized and temporary E & S controls have been removed as required. Overall, the footprint of site surface has been reduced according to the plans and looks good. I recommend the Well Site Restoration Report and Notice of Termination be approved. No violations noted.
