2274685 Primary Facility 2014-05-21 Complaint Inspection No Violations Noted On 5/21/2014 Ms. Renner and I inspected the site. Severe erosion was noted on the cut slope of the infiltration basin and at the inlet swale entering the basin. In addition, erosion was noted in the roadside swale above the basin. Based on the erosion I recommend re-evaluating the basin design itself in addition to possibly adding additional cross culverts to reduce the volume of stormwater entering the basin.

Bush Hill Bennett 1H: well head capped, cellar empty. Annular vents were monitored for combustible gas using an Altair 5X meter calibrated on 01/04/16. No gas was detected. There were no violations at this time. Left location @ 14:30.

2472501 Primary Facility 2016-04-05 Drilling/Alteration No Violations Noted Weather: cold, clear. Arrived @ 7:30. Well being blown down as part of a plan to manage intermediate casing pressures for top-holed wells on this pad. At the time of my inspection the well was blowing down to a gas buster and bringing up mainly gas with some fluid and mud. A gauge on the flowing 2” vent for the 9-5/8” casing read between 85 and > 100 psi on a 0-100 analogue gauge. Contacted Dale Fidurko who said the gauge was broken. Previously pressure had settled down to 20 psi on a 0-6000 psi gauge. The replacement gauge was more sensitive, but didn’t work. Discussed flaring the well with Dale who said that a flare stack was available at Inflection’s yard on Rt 87 and would be brought on site and connected up. Left location @ 10:00

14:30, returned to Bennett. Infomed by Dale by phone that flaring ended at ¿ 12:30.At the time of my inspection, met with Dale Fidurko. Blind flange previously capping the well was replaced with a taller, valved cap. 9-5/8” casing pressure ¿ 100 psi on 0-5000 psi analog gauge. Inflection will check the casing pressure daily. It is anticipated that pressures will rise to ¿ 1400 psi between 4/7/16 and 4/20/16 when the next stage of the study is expected to begin. If pressures should rise higher, other options will be considered. Left location @ 15:00.

2473237 Primary Facility 2016-04-11 Drilling/Alteration No Violations Noted Weather: cold, cloudy, rain. Arrived @ 08:00. Received message from Dale Fidurko on 4/09/16 @ 9:34 informing the Department that shut-in pressure on the Bush Hill Bennett 1H was 1900 psi and Inflection would flare to reduce pressure. Inflection flared from approximately 10:00 to 12:30 on 4/09/16, afterwards shut-in. On 4/11/16, at the time of my inspection Brian Twigg (Inflection ) checked on the well’s shut-in pressure which was 2050 psi. While I was onsite, Dale Fidurko called to tell me that later in the morning, Inflection would begin flaring the well again to reduce the pressure. Noting that the pressure was building to greater than 1800 psi in less than a day, Dale suggested that the well be continuously flared and I agreed with his suggestion. Left location @ 09:00
2473837 Primary Facility 2016-04-12 Drilling/Alteration No Violations Noted Weather: sunny & windy, following earlier rain. Arrived @ 09:30. Dale Fidurko called earlier in the morning to say that the well is being flared, with no back pressure, after a blow-down of approximately 1.5 hours. At the time of my inspection the well was being flared and there was no back pressure.

The annular vents were monitored using an Altair 5X gas meter calibrated on 4/12/16. No methane was detected in either annulus. Left location @ 09:45.

The two other top-holed wells: Chaapel Mountain Bennett 1H and Chaapel Mountain Bennett 3H have been shut-in, but pressure monitoring for these two wells also began on 4/25/16 prior to a pressure and flow monitoring study. BHB 1H: shut-in. Intermediate casing pressure: 1600 psi, (0-3000 psi analog gauge). BHB 2H in production. Chaapel Mountain Bennett 1H: Intermediate casing pressure: 1070 psi (0-3000 psi analog gauge). Chappel Mountain Bennett 3H: Intermediate casing pressure: 920 psi (0-3000 psi analog gauge).

Plan is to begin flowing the wells on 4/28/16 and begin measuring rates.

2492577 Primary Facility 2016-06-09 Drilling/Alteration No Violations Noted Arrived @ 10:45. Weather: sunny, pleasant. Personnel on site: Brian Twigg (Inflection), Matt Caldwell (Moody). The purpose of this inspection was to check on the monitoring/flaring arrangements for the 3 top-holed wells on location: BHB 1H, CMB 1H, and CMB 3H, following receipt of Inflection’s update on Intermediate Casing Pressure management for these wells. At the time of inspection: BHB 1H was flaring, and CMB 1H and CMB 3H were shut-in, with intermediate hut-in pressures at 200 psi. Continuous pressure loggers were fitted to the intermediate casings of each top-holed well. Beginning 6/14 @ 6:00, the BHB 1H would also be shut-in. Pressures on the CMB 1H and CMB 3H would be monitored to determine whether a hydraulic connection between the BHB 1H and the CMB wells exists at depth. No violations were observed during this inspection. Left location @ 11:20.

This locaton was previously hit by historically heavy rain which damaged the access road. The BHB 1H was in production. Pressure at the well was 248 psi (digital). No combustible gas was detected at the annular vents. Left location @ 14:10.

One GPU within containment. No violations taken at this time.
