On 8/29/2017, Doug Kepler (Seneca)contacted me at 12:59 concerning a release of produced water from a flow line adjacent to the production tanks on the site. Mr. Kepler reported that the produced water line washed-out at a 90 (degree angle) and was discovered at approximately 12:00. At 13:18, I called Kyle Kline (Seneca) to discuss the release. Mr. Kline reported the surface area of the release was approximately 15'X20'. Mr. Kline estimate the volume of the release to be about 1 barrel. I asked Mr. Kline to call after the cleanup began to evaluate the volume estimate and to coordinate a site meeting. Mr. Kline called at 16:38 and said the excavation was almost complete. We agreed to meet the following day.

At 12:15 on 8/30/2017, I arrived onsite and met with Mr. Kline. Onsite I found the breached pipe was fixed and an excavation about 20'X20' approximately 5 feet deep was open around the pipe. A "spoils pile" was located adjacent to the excavation which was allegedly not

Clark Smith (DEP) and I entered the site to evaluate the excavation and remediation if the release reported to the Department on 8/29/2017. Field soil conductivity was between 90-1,650 umhos/cm throughout the entire excavation. The area depicted in photo #2 had the highest conductivity. This small area was also the only area over 1,000 umhos/cm anywhere in the excavation or in the area adjacent to the excavation. Mr. Smith discussed the sampling requirements with the operator before our site inspection. The consultant provided field screening and confirmation sample figures(see photos). The Department is awaiting confirmation sample results. Previous violations cited to ESX12-081-0138 remain outstanding, no new violations noted.
