2482645 Primary Facility 2016-05-06 Routine/Complete Inspection No Violations Noted I entered the location at 09:37 and began the inspection. At the time of the inspection, the Flicks Run 7H (081-21114), 8H (081-21115), 9H (081-21116), and 10H (081-21117) were in production. Two other well permits remain active for this site but have not been drilled. The site currently lacks vegetation on the southern fill slope and on the southeast side of the topsoil stockpile on the south side of the pad. The well site remains at its originally constructed size and has not been restored but the three rock infiltration sumps designed to handle the stormwater based on the PCSM (Post Construction Stormwater Management) plan have been installed. The site is not yet required to be restored and the revised restoration assessment received by the Department acknowledges this by categorizing the site as having an active well permit. The current ESCGP expires on 5/28/2017. No Violations Noted.
2662556 Primary Facility 2017-11-20 Routine/Complete Inspection No Violations Noted Weather 38 Deg. F, sunny and windy. Well has a digital gauge with 317 PSI production pressure. Also backside pressure on the tubing digital gauge shows 564 PSI. 0% combustible gas detected at the time of inspection. No annulus vents observed during inspection.
2681089 Primary Facility 2018-01-03 Routine/Complete Inspection No Violations Noted The access road from the Loch pad to the Flicks Run East well pad itself was in good condition and well maintained. A few inches of snow was covering the entire site. Aggregate material was stockpiled onsite for winter maintenance. On the well pad, four wells were in production. Sand separators remain installed at the production units and both (for each well) are in containment. No staining was visible around any of the production equipment. Methanol was stored (covered and in containment) near the wellheads for injection.

The ESCGP permit has expired and Post Construction Stormwater Management (PCSM) has not yet been permitted or implemented. I recommend re-permitting the site and following the COA which indicates this site will be restored in 2019. No violations noted.

2802024 Primary Facility 2018-11-13 Routine/Complete Inspection No Violations Noted Fours wells staged for production phase. Four each sand and water separators within containment. No leaks detected from production equipment. Methanol tank located near well heads. No violations noted.
