2554398 Primary Facility 2017-01-12 Routine/Complete Inspection No Violations Noted Arrived @ 14:30. Weather rainy, temperatures in low 50s. There were 7 wells spudded on site in a linear array. The wells were tagged with well names and permit numbers. An Altair 5X gas meter calibrated on 01/05/17 was used for field monitoring of combustible gas.

Stunner Smith 2H (SS 2H), 081-21018: top-holed-only with nightcap. Three annular vents (2 – 13-3/8” x 9-5/8” vents and 1 – 20” x 13-3/8” vent). Methane was not detected at any of the vents. On 2/8/16 Inflection monitored pressure on the 9-5/8” casing. Pressure was 325 psi, and blew down in 15 minutes. There were no violations for the Stunner Smith 2H. Left location @ 14:50.

2696674 Primary Facility 2018-02-22 Follow-up Inspection No Violations Noted Arrived at 7:40. Weather: rainy, temperatures in low 40's. There are 7 wells. The Stunner Smith 1H and Ryder Park 1H are in production. Stunner Smith 2H and 3H are top-holed. SS 2H: TD 6692' with a plug from 6648' to 6144'. IC casing to 3018'. SS 3H: TD 6621', IC casing to 3007. This is a follow up IR to see if methane is still leaking from the join between the intermediate vent pipe and the intermediate casing. It was. Will schedule follow-up with Inflection. For the SS 6H, a plugged conductor, all that was visible was a gravel filled cellar. For the SS 4H and SS 5H, also plugged, the cellars were filled with water.
2700016 Primary Facility 2018-03-01 Follow-up Inspection No Violations Noted Arrived at 11:00Weather: warm overcast. For the SS2H and SS3H, Previously methane had been leaking from the join between the intermediate vent pipe and the intermediate casing. This is a follow-up with Landon Vanorn and Brian Twigg to check on the pressure build after 66-1/2 hrs and obtain isotopic samples. SS 2H: final pressure: 37-1/2 psi on 0-100 analog gauge. Gas isotopic sample number: 20180301237102G acquired at 11:10. SS3H: final pressure: 5-1/2 psi on 0-100psi analog gauge. Gas isotopic sample: 20180301237103G.
2806662 Primary Facility 2018-11-21 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging, inspection of the 2H and 3H original boreholes was conducted November 21, 2018. The operator has recently submitted a Certificate of Plugging for each of these original boreholes after both were plugged prior to reaching intended TD as part of a plug and skid event. At the time of inspection, the operator was on location. I inspected the wellbore, annular vents, and cellar of each well. We discussed the marker requirements on a plugged well; each well marker may say "Original borehole" to help clarify any future confusion. No obvious issues were observed at surface during this inspection.
2817871 Primary Facility 2018-12-17 Routine/Complete Inspection No Violations Noted Weather was 30° and cloudy at time of inspection. Met with Inflection representative Ross Drnjevich on location. Rig was currently in the process of reaming the hole for the surface casing clean to run surface casing. They were then planning to skid the rig and cement the 6H and 2H surface casings in the morning on 12/18/18. Conducted a walk around inspection of location. No violations were noted at time of inspection.
2821520 Primary Facility 2018-12-20 Routine/Complete Inspection No Violations Noted Weather was 30° and cloudy at time of inspection. Met with Inflection representatives Ross Drjnevitch and Greg Malekos on location. Well surface casing was cemented on 12/19/18. Reviewed the cement job logs and no discrepancies were noted. Discussed the cement jobs with the Inflection representatives and no discrepancies were noted. Well was closed in with a night cap and the rig skidded onto drill surface casing for other wells on the pad. No violations noted at time of inspection.
2822720 Primary Facility 2019-01-03 Routine/Complete Inspection No Violations Noted Weather was 38° and cloudy at time of inspection. Met with Inflection representatives Jamie Czerneski and Greg Malekos on location. Rig had just finished drilling out the shoe track of the 18 5/8" casing at time of inspection. Rig was in the process of tripping out of the hole to pickup a directional bottom hole assembly(BHA). Rig will continue drilling the hole for the intermediate casing. No violations were noted at time of inspection.
2827936 Primary Facility 2019-01-15 Routine/Complete Inspection No Violations Noted Weather was 25° and sunny at time of inspection. Met with Inflection representatives Ross Drjnevitch and Greg Malekos on location. Well recently had the first intermediate string of 13 3/8" casing drilled and cemented. Well was closed in at time of inspection with a nightcap. Water in the cellar was frozen solid at time of inspection. No violations were noted at time of inspection.
2835452 Primary Facility 2019-02-05 Routine/Complete Inspection No Violations Noted Weather was 37° and sunny at time of inspection. Met with Inflection representatives Greg Malekos on location. Well recently had the first intermediate string of 13 3/8" casing drilled and cemented. Well was closed in at time of inspection with a nightcap. Water in the cellar was frozen solid at time of inspection. No violations were noted at time of inspection.
2840486 Primary Facility 2019-02-14 Routine/Complete Inspection No Violations Noted I met Ross Drnjevich of Inflection on the site. H&P 521 on location, drilling the 2HN well at 8300'. The chemical containment appeared to be in good condition, but also contained several inches of snow and ice. The secondary containment for the mud tanks had been bolted on with wood strips on the inside of the containment in the back, approximately 4-5" down from the top. Although it is unlikely that this brought the containment volume below 110% of the volume of the tanks, I still recommended to Ross that these liners be attached on the outside of the berms. Meltwater had built up to 3-4" deep on the backside of the pad, and Ross told me he would have it vacced.
2842010 Primary Facility 2019-02-20 Routine/Complete Inspection No Violations Noted Weather- 21 degrees and cloudy

Met with Inflection representatives Ross Drnjevich and Greg Malekos on location. This well recently had its second intermediate casing drilled and cemented to a TD of 8184'.
