2058470 Primary Facility 2012-04-10 Drilling/Alteration No Violations Noted
2061823 Primary Facility 2012-04-18 Routine/Complete Inspection No Violations Noted DeSoto #8 onsite on the 1H, Slumberger logging. site looks clean, good containment. No E&S issues
2062528 Primary Facility 2012-04-25 Routine/Complete Inspection Viol(s) Noted & Immediately Corrected
2089670 Primary Facility 2012-08-06 Routine/Partial Inspection No Violations Noted
2094047 Primary Facility 2012-08-21 Drilling/Alteration No Violations Noted
2113376 Primary Facility 2012-10-24 Routine/Complete Inspection No Violations Noted Frac'ing the pad, on the 3rd stage of the 4H
2264655 Primary Facility 2014-03-26 Routine/Complete Inspection No Violations Noted
2412790 Primary Facility 2015-09-28 Routine/Complete Inspection No Violations Noted Inspection on wells in production. No violations noted at this time.
2482615 Primary Facility 2016-05-06 Routine/Complete Inspection No Violations Noted At the time of the inspection, the Gayla Loch 1H (081-20909), 2H (081-20952), 3H (081-20910), 4H (081-20911), and 5H (081-20912) were in production. The well site has not been restored and the sedimentation basins have not been converted to infiltration basins based on the PCSM (Post Construction Stormwater Management) plans. Rock Infiltration sumps along the access road have been installed as designed in the PCSM plans. The revised restoration assessment acknowledges the well site has not been restored and drilling ended on 5/5/2014. The well pad remains at its original size and no well permits have been issued since 2012. The well site has not been restored based on the criteria above and the current ESCGP expires on 12/6/2016. I recommend restoring the well site or implementing the PCSM and submitting a 2-year extension. Overall, the site is currently permanently stabilized (with both vegetation and rock) but has not been restored per §3216(a) of the Oil and Gas Act. No Violations Noted.
2662554 Primary Facility 2017-11-20 Routine/Complete Inspection No Violations Noted Weather 38 Deg. F, sunny and windy. Well has a digital gauge with 323 PSI production pressure. Also backside pressure on the tubing digital gauge shows 377 PSI. 0% combustible gas detected at the time of inspection. No annulus vents observed during inspection.
2681121 Primary Facility 2018-01-03 Routine/Complete Inspection No Violations Noted The access road (including stream crossings and drainage features) from SR 184 to the well pad itself was in good condition and well maintained. A few inches of snow was covering the entire site. On the well pad, five wells were in production. Sand separators remain installed at the production units and both (for each well) are in containment. No staining was visible around any of the production equipment. Methanol, foamer, and defoamer were stored (covered and in containment) near the wellheads for injection.

The ESCGP permit has expired and Post Construction Stormwater Management (PCSM) has not yet been permitted or implemented. I recommend re-permitting the site and following the COA which indicates this site will be restored in 2019. No violations noted.

2793634 Primary Facility 2018-10-24 Routine/Complete Inspection No Violations Noted Five wells staged for production phase. Five each sand and water separators. No leaks detected from production equipment. Methanol tanks located near well heads. No violations noted.
