The infiltration basins clearly do not infiltrate well and do not dewater within 72 hours. Erosion is still visible in the channels leading into the basins. This sediment is obviously a contributor to the reduced infiltration rate. In addition, the south/southeast side cut-slope has not achieved 70% vegetative cover. This site does not meet the PCSM criteria outlined in the approved plans and requires corrective action. Please correct the erosion in the channel leading to the infiltration basins ASAP.
2514930 Primary Facility 2016-08-24 Site Restoration No Violations Noted Notice of Termination/Well Site Restoration Report Inspection; 8/24/2016.

I entered the site at 14:11 to find one well in production. The PCSM (Post Construction Stormwater Management) plan calls for the rock-lined ditch on the cut side of the well pad to be converted into an infiltration trench yet the current trench is filled with rock and has the same erosion control blanketing originally used for stabilization. In addition, the calculations describe the infiltration basins a being empty after rain events. The basins currently onsite do not dewater and appear to have limited infiltration capacity. I recommend implementing the infiltration trench as designed and re-evaluating the infiltration basins. In addition, the slopes above the infiltration basins are not permanently stabilized. I recommend denying both the Notice of Termination the Well Site Restoration Report. No Violations Noted.

The PCSM (Post Construction Stormwater Management) plan has established a infiltration trench on the fill side of the well pad.

2841887 Primary Facility 2019-02-05 Routine/Complete Inspection No Violations Noted XTO Energy Inc. has submitted a notice of termination (NOT) to the Department for Buck Unit A covered under ESX110810076(01). The Department received the NOT on January 07, 2019.

Department representative Jared Jacobini conducted a restoration site inspection at the above listed site on February 5, 2019. The site sign, emergency response plan, and ESCGP were onsite for review.

The Infiltration detail does not show or describe corrugated pipe (Photo 8) present inline with underdrain on the downstream side of embankments in the infiltration basin.

The department recommends the following:

Add riprap to Infiltration Basin #2 (Photo #15) due to a large amount of flowing water causing erosion in the area that PC4 enters the basin. Fill and seed in the area of tire ruts (Photo #13-14). Fill and seed settling areas at each basin (Photo #10, 12). Determine if the infiltration basin is functioning (Photo #11). Monitor PC4&PC5for erosion (Photo #2, 16). Removing construction materials present in Infiltration Trench
