2325621 Primary Facility 2014-11-25 Incident- Response to Accident or Event No Violations Noted On 11/24/2014, Zach Subra reported a release of flowback from failed tubing on a coiled-tubing rig. The release was approximated at 4-5 barrels. Since the flowback was sprayed from the tubing most of the release captured on containment while less than 1 barrel sprayed on the pad surface. According to Range Resources, the impacted pad material was excavated, placed into two roll-offs, and staged at the landfill (until sample results are returned).On 11/25/2014, I met with Irvin Gleason and Zach Subra (Range) and we walked the site. The release occurred on the northeast side of the well pad. Field conductivity in puddles onsite ranged from 500 umhos/cm to 1110 umhos/cm off containment. Water ponded on containment had field conductivity of 3,140 umhos/cm and had friction reducer floating on the surface. I asked that they continue to vacuum the flowback from containment and the one puddle with slightly elevated conductivity. Violations Noted and Immediately Corrected.
2447773 Primary Facility 2015-12-23 Site Restoration No Violations Noted Well Site Restoration Report Inspection; 12/23/2015. At the time of the inspection, the two wells onsite were in production. The well site is well vegetated except for the formation of a small erosional channel leading into the sedimentation basin on the northeast side of the well pad. The site is not permanently stabilized and does not meet the PCSM plans or the restoration requirements. I recommend the WSRR be denied until the above concerns can be addressed. The Well Site Restoration Reports for the Sechrist Mark Unit 1H (081-20760) and Sechrist Mark Unit 2H (081-21301) contain no waste disposal information and are considered incomplete. No Violations Noted.

At the time of the inspection, the two wells onsite were in production. In addition, Hawbaker was onsite installing PCSM facilities which include two infiltration basins (with under-drains). E&S controls were installed and functioning properly. The construction of the PCSM features is being performed based on a modification to the ESCGP and a 2-years restoration extension which were both approved on 6/14/2016. The plans also call for a portion of the well pad surface to be reduced in size. Overall, the site is in good condition. No Violations Noted.

Two wells were in production at the time of the inspection. PCSM plans have been implemented but the site is not permanently stabilized (see photos). I recommend denying the Notice of Termination for the E&S permit. No violations noted.
