Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2039544 Primary Facility 2012-02-02 Drilling/Alteration No Violations Noted
2077380 Primary Facility 2012-06-20 Routine/Complete Inspection No Violations Noted

2467510 Primary Facility 2016-03-22 Routine/Complete Inspection No Violations Noted
2528410 Primary Facility 2016-09-29 Routine/Complete Inspection No Violations Noted Routine inspection of unconventional well in production. No violations noted at time of inspection.
2685789 Primary Facility 2018-01-24 Routine/Complete Inspection No Violations Noted Weather was 26° cloudy and windy at time of inspection. Well has an analog gauge above the swab valve on the production tubing, on the 5 1/2 casing and on the 9 5/8 x 5 1/2 casing annulus. The tubing pressure was approximately 500 psi, casing was approximately 450 psi and the annulus pressure was approximately 0 psi at time of inspection. Cellar was frozen solid at time of inspection. No other annulus vents were plumbed to surface for observation.
2780298 Primary Facility 2018-09-26 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection. E&S inspections are being completed and logged onsite. The emergency response plan was also available.

There are 4 properly tagged Marcellus wells staged for production. There are 4 GPUs onsite and in secondary containment. There are also 3 produced water tanks in secondary containment.

The areas around the well pad are >70% vegetative cover and densely wooded. Compost filter sock (CFS) has been used to construct PCSM infiltration berms. The CFS berms have been seeded but some areas remain bare.

Overall the site appears stable and no E&S issues were observed.
