I entered the location at 10:18 and began the inspection. At the time of the inspection, the Wallis Run Hunting Club A 2H (081-20669), 4H (081-20670), 5H (081-20961), and 6H (081-20971) wells were in production. The well pad area and access road are stabilized and no erosion was noted. The well site remains at its originally constructed size and has not been restored. The ESCGP expired on 6/30/2015. The revised restoration assessment indicates this site will be re-permitted sometime in 2016 and those revisions will be implemented sometime in 2017.

The Department considers a well site to be restored under Section 3216(c) of the 2012 Oil and Gas Act when the oil or gas owners or operators meet the following:

1. All permanent post construction stormwater control features as identified in the Post Construction Stormwater Management/Site Restoration Plan are in place.

2. Remaining impervious areas are minimized. See Section 102.8 (b)(4). Impervious areas include, but are
