2625404 Primary Facility 2017-08-03 Site Restoration No Violations Noted Notice of Termination/Well Site Restoration inspection. Three wells are in production and the site has been restored according to the Site Restoration plans. The well site remains at the originally constructed size but justification was provided in the permitting process to keep the current size to accommodate production and maintenance of the wells as wells (including a workover/service rig). I recommend the Well Site Restoration and Notice of Termination for this site be approved. No violations noted.
2791549 Primary Facility 2018-10-19 Routine/Complete Inspection No Violations Noted Three wells stage for production. Three each sand and water separators within containment. No leaks detected from production equipment. No violations noted.

One regulatory inactive well on location.

2854456 Primary Facility 2019-03-15 Routine/Complete Inspection No Violations Noted A routine (pre-plugging) inspection of this uncompleted well was conducted March 15, 2019. Records indicate this well has (cemented) surface casing only set. There is no indication float equipment was drilled out and therefore any formation below the casing seat is isolated with cement by shoe and shoe track. The operator intends to satisfy the regulatory requirements outlined in the plugging regulations by filling the inside of the surface casing with a non-cementing material (gravel) to surface. At the time of inspection, access to the well was limited due to a metal cover placed over the well cellar. The well could be seen somewhat through a small hole in this cover. There was no audible or visible venting, bubbling, discharge, or other obvious issues observed. No gas was detected in the cellar with an Altair 5x gas meter. The operator has submitted a Notice of Intent to plug this well, and the operation is tentatively scheduled for next week.
