2236985 Primary Facility 2014-01-14 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected Violations pending for 081-20517 (6H) well. produced water spill to the pad. Remedial Action Report approved on 3/17/2014.
2465515 Primary Facility 2016-03-16 Routine/Complete Inspection No Violations Noted At the time of the inspection, three wells were in production while the fourth (081-20518) was granted inactive status on 10/29/2013. The well site is currently well vegetated and shows no signs of erosion. The well site remains at the originally constructed size and has not been restored as indicated in the permitted plans. The requirement under Section 3216 (c) of the Oil and Gas Act requires that "within nine months after completion of drilling of a well, the owner or operator shall restore the well site."

The approved plans call for a portion of the well site to be scarified and production facilities are installed where the site is to be scarified. I recommend re-permitting the site to incorporate a feasible restoration of the site. No Violations Noted

2524913 Primary Facility 2016-10-03 Routine/Complete Inspection No Violations Noted Arrived @ 14:30. Weather: windy and cloudy. There were 3 producing wells and 1 plugged well in a linear array on location. Lease operator Kevin Barr and automation tech Matt Musgrave were onsite when I arrived. Water injection and plunger systems were in use on the three production wells to limit salt build-up. The water injection plumbing was covered in silver insulation material, gas and water were plumbed to the GPUs.

A-6H, in production. Cellar water-filled, no bubbles. Top gauge: ¿ 900 psi, (0-10,000 psi, analog). One annulus, shut-in, 3psi (digital).

2698683 Primary Facility 2018-02-27 Routine/Complete Inspection No Violations Noted Weather was 36° and sunny at time of inspection. There were no ARD representative on location at time of inspection. Well had a digital gauge on the tubing, the production casing and the 5 1/2 x 9 5/8 annulus vent. Tubing pressure was 804 psi, casing was 823 and the annulus pressure was 2 psi at time of inspection. There were no other annulus vents plumbed to surface. No violations were noted at time of inspection.
2854726 Primary Facility 2019-03-18 Routine/Complete Inspection No Violations Noted Weather fair @ 39°F. Four wells on location and three wells staged for production. Well 081-20518 is granted regulatory inactive status. Three GPUs within polyurethane/foam containment. No leaks detected. No violations noted.
