I entered the location at 13:55 and began the inspection. At the time of the inspection, the four wells onsite are on inactive status (081-20501, 081-20502, 081-21011, and 081-21012). The well pad surface was overgrown but in good condition and no E&S issues were observed. The site has no active well permits that have not been drilled.

ESX11-081-0127 expires on 11/2/2016. The well site has not been restored and PCSM requirements have not been met. Anadarko’s most recent restoration timeline submission indicates PCSM will be permitted in 2016 and installed in 2017.

The Department considers a well site to be restored under Section 3216(c) of the 2012 Oil and Gas Act when the oil or gas owners or operators meet the following:

1. All permanent post construction stormwater control features as   the Post Construction Stormwater Management/Site Restoration Plan are in place.

2. Remaining impervious areas are minimized. See Section 102.8 (b)(4). Impervious areas include, but

We entered the site and talked to Merek Romatowski (Company Man). Mr. Romatowski said they were under pressure on stage 22 of 26 on the Harry Stryker A 2H (081-20501). The containment at the ramps over the containment berm has been damaged and has a few small holes but has clearly been patched several times already. I recommend continuing to monitor and repair containment as needed. A soft spot under containment was noted southeast of the wellheads. I recommend monitoring this area as it does appear to be in the travel lane on containment and more susceptible to damage. No violations noted.

2759771 Primary Facility 2018-08-01 Routine/Complete Inspection No Violations Noted I met Rodney Blanton of ARD when I arrived. The frac was active, with 17 of 26 stages completed on the 6H well. BJ Services was performing the frac. I walked the site with Rodney, testing conductivity in the numerous puddles on the pad and locations on containment. Puddles on the pad were from .35-.675 mS/cm, and water on containment was from .15-1.5 mS/cm. Rodney informed me that he had a vac truck working at the blender constantly to clean up the area. Sitewide and secondary containments appeared to be in good condition.
2831720 Primary Facility 2019-01-25 Routine/Complete Inspection No Violations Noted Weather was 21°, sunny and windy at time of inspection. There were no Alta representatives on location at time of inspection. There was no wind sock on location at time of inspection. There was a west wind blowing an estimated 10-15 mph. No odors were noted anywhere on location at time of inspection. There were 2 trucks on location that were hauling away produced water at time of inspection. There was no visible evidence of any spillage during transfer of water from the tanks to the trucks. Walked around location with an Altair 5x multi-gas meter that was calibrated on 1/23/2019. 0% combustible gas was detected anywhere on location at time of inspection. Well had digital pressure monitoring equipment attached to the production casing, the production tubing, and the production annulus vent. Tubing pressure was 1597 psi, casing pressure was 2153 psi and annulus pressure was 1251 psi at time of inspection. There were no other annulus vents plumbed to surface. No violations were noted at time of inspection.
