2435534 Primary Facility 2015-12-04 Routine/Complete Inspection No Violations Noted I arrived onsite at 09:51. Five of the six wells on the pad were in production. The tank pad (located to the southwest of the well pad) remains in place yet no work has been done on any of the wells since July 2013. According to the well records, drilling on the last well (081-20492) was completed on 12/7/2011 and there are no other active well permits for this well site. Section 3216 (c) requires that "within nine months after completion of drilling of a well, the owner or operator shall restore the well site." The Well Site Restoration assessment summary indicates that a new well permit was to be submitted to the Department Fall 2015. As of the date of this inspection report, no well permit applications are pending or have been issued according to eFacts. In order to bring the well site into compliance, the Department recommends that EXCO restore the well site in accordance with Section 3216(c) or comply with the restoration schedule as submitted.
2492509 Primary Facility 2016-06-09 Routine/Complete Inspection No Violations Noted Arrived @ 09:00. Weather sunny and warm. No company representatives were present at the time of the inspection. Two of the six wells on site (4H and 5H) were in production.

Warner Unit 6H: Gravel cellar. Top gauge: 250 psi (digital), 5-1/2” gauge: 0 psi (digital). Annular vents (13-3/8” x 9-5/8” and 9-5/8” x 5-1/2”) were monitored for methane with an Altair 5X gas multimeter (calibrated on 5/23/16). No gas was detected. No violations were observed at the time of this inspection. Left location @ 09:30.

2568250 Primary Facility 2017-03-06 Routine/Complete Inspection No Violations Noted Routine Inspection; 3/6/2017.Dan Harvey (DEP) and I arrived onsite at 09:00 and met with the landowner. Five of the six wells on the pad were in production. The tank pad has been removed and the site has been restored in accordance with the ESCGP which was upgraded to include Post Construction Stormwater Management (PCSM). The site is well vegetated but has not quite reached >70% uniform perennial vegetative growth.

We discussed the PCSM features and maximizing the amount of area that can be restored to crop production. A long rock pile and a steep water bar at the northwest corner (near the entrance off SR118) of the well pad have not been restored back to approximate original contours. I recommend moving the rock pile onto the well pad and smoothing the water bar/soil stockpile so that the area can return to agricultural production. The landowner and the operator have already coordinated on the removal of silt sock on the site so that the fields can be returned to agricultural production. No Violation
