2478324 Primary Facility 2016-04-19 Routine/Complete Inspection No Violations Noted At the time of the inspection, the Kensinger Unit 3H (081-20255), 6H (081-20388), 7H (08-20377), 8H (081-20378), 9H (081-20379), 10H (081-20380), and 11H (081-20381) wells were all in production. The well site remains at its original size and has not been restored. The Well Site Compliance Information Request reply (submitted 9/21/2015) states the well pad was reclaimed and the NOT will be submitted in 9/2015. The Department has no record of an ESCGP issued for the site and no well permits have been issued for the site since 2010. According to the Department’s records, the wells onsite were drilled in 2010 and 2011 and completed in 2010 and 2012. The well pad remains at its original size although aerial photos indicate a pit was located onsite that has since been closed. I recommend you apply for an ESCGP-2 to cover this since it has not been restored and is part of a larger disturbance.

The inspection was prompted when Bird Smith called on 2/13/2017 to report a produced water leak. According to the call, approximately 12 barrels of produced water leaked from the Kensinger Unit 3H (081-20255) production tank into containment due to a frozen pipe. Ice was covering the bottom of the containment at the time of the release. At the time of the inspection, seven wells (081-20255, 081-20388, 081-20377, 081-20378, 081-20379, 081-20380, 081-20381) were in production. Field conductivity was 1,500 umhos/cm in containment on top of the ice just under the leak. Under the ice, the water was 120 umhos/cm. The area around the containment was frozen with snow but no evidence was found to suggest produced water left containment. On 12/21/2016, the Department issued an ESCGP for this site for the first time. I recommend implementing the PCSM/restoration plans for the site as soon as possible. No Violations Noted.
