2437879 Primary Facility 2015-12-30 Routine/Complete Inspection No Violations Noted Arrived @ 13:15. Weather: foggy, cloudy, some fine drizzle5 wells, formerly in production, were shut-in. All well cellars were filled with gravel. There were two open annular vents at each well. The 13-3/8” x 9-5/8” was vented with a 1” pipe. The 9-5/8” x 5-1/2” annulus was vented with a 2” pipe. The annuli were monitored with an Altair 5X gas meter calibrated on 12/28/15.
2543938 Primary Facility 2016-12-08 Routine/Complete Inspection No Violations Noted Routine Inspection; 12/8/2016. I entered the location at 10:06 and began the inspection. At the time of the inspection, the Bower Unit 1H (081-20149, 2H (081-20361), 3H (081- 20362), 4H (081-20456), and 6H (081-20356) wells were in production. The well pad area was well vegetated and can be considered permanently stabilized. The well site has not been restored and the Well Site Compliance Information Request reply (submitted 9/21/2015) says the pad was reclaimed and the Notice of Termination was to be submitted in September of 2015. The well pad remains at its original size and the site currently does not conform to the permitted restoration plan. The original E&S permit has been expired since 10/16/2014.

The Department considers a well site to be restored under Section 3216(c) of the 2012 Oil and Gas Act when the oil or gas owners or operators meet the following:

2694616 Primary Facility 2018-02-14 Routine/Complete Inspection No Violations Noted Weather was 30° and cloudy at time of inspection. Well had a digital pressure gauge attached above the swab valve for the tubing and attached to the 5 1/2 casing valve. Pressure on the tubing was 259 psi and the 5 1/2 casing pressure was 322 psi at time of inspection. There were three annulus vents plumbed to surface. The 5 1/2 x 9 5/8 annulus vent was plumbed across location to the produced water tanks. The 9 5/8 x 13 3/8 and 13 3/8 x 20 annulus vents were open and 0% combustible gas was detected at time of inspection. No violations were noted at time of inspection.
2833647 Primary Facility 2019-01-28 Routine/Complete Inspection No Violations Noted On January 28, 2019, Department representative Brad Kline conducted a routine site inspection at EXCO Resources, LLC. Bower Pad 1H. Site inspection log was onsite for review. No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover.

There are a total of 5 producing wells located on this well pad. All wells are tagged with API/well permit numbers. Well cellars are filled in with gravel. There are 5 GPU’s and 4 steel vertical production fluids tanks onsite. Production fluid tanks were stationed in secondary containment.

No violations noted. The Department recommends submitting a Notice of Termination and a Well Site Restoration Report when the criteria is met.
