2442777 Primary Facility 2016-01-20 Routine/Complete Inspection No Violations Noted : Arrived @ 11:55. Temperatures in the 20’s, calm and cloudy with a light coating of snow. There was one well on location -- tagged and set up for production but shut-in. The cellar was filled. The 9-5/8” x 5-1/2” annular vent gave a field reading of 100% methane, with no appreciable flow (Altair 5X gas meter calibrated on 1/20/16). There were no violations at the time of inspection. Left at 12:15.

I entered the site at 12:10 off existing hunting club roads. At the time if the inspection, one well was in production. PCSM is permitted but has not yet been implemented and the site is not restored. The site is well vegetated and no E&S issues were noted. No violations noted.

2669626 Primary Facility 2017-12-11 Routine/Complete Inspection No Violations Noted Weather is 28° and snowy at time of inspection. There were two annulus vents plumbed to the surface by a 2 inch pipe with 90° elbow's on them. They were not labeled. There was an analog pressure gauge attached to the pay line that read approximately 200 psi. 0% combustible gas was detected on either vent or in the cellar area at time of inspection.

At the time of the inspection, one well was in production. Post Construction Stormwater Management (PCSM)/ Site Restoration plans have been recently implemented. The site is stabilized with straw mulch and erosion control blanketing. No violations noted.
