The well has never been produced but was drilled in 2009. Another well was previously permitted onsite but expired on 11/16/2016 without being drilled. The Department has not received a Well Site Restoration Report for the Ultimate Warrior 1 well (081-20178) which should have been received in October of 2010. The well and well site were owned by Rice Energy at the time the WSRR was originally due. In addition, since the site has not been restored and pipeline has been permitted, the scope of the project has become large enough to require the well site to be covered by an ESCGP. I recommend permitting the site and implementing the plan or requesting a 2-year extension. No Violations Noted.
