2509667 Primary Facility 2016-08-11 Routine/Complete Inspection No Violations Noted Inspection of the Grover Burkhardt (081-20009) abandoned gas well in Lycoming County was conducted on August 11, 2016 at 11:15 am. The well is on the DEP Abandoned list and has no plugging certificate or record of being plugged. The original well record shows they drilled down to the Tully, 5429-5665’, may have TD’d there or may have drilled deeper but the Well Record is old and washed off, including the date drilled. A more recent Well Record exists from 1976 which is more legible. This well record shows there being 997’ of 8 5/8” casing set. Looking at the well record, it looks like they knocked a wooden plug down to 4533’ in the open hole section so they can run electric log for gas (“no gas encountered). The bottom of the well record says “This is a Pre Act Well.” No cementing information is on the well record and the casing may not be cemented. At the time of inspection: The landowner’s brother was working in the shop, came out, and directed me to the well. The casing came up above ground level and was
2837119 Primary Facility 2019-02-07 Administrative/File Review No Violations Noted The purpose of this admin inspection is to document new findings while reviewing well records on this legacy well on the DEP plugging list. When looking at the newer well record on file, the one that was submitted by the company that re-entered the hole for logging,

it is misleading and looks like TD is at 5664', where they call the Tully Formation. However, after discovering the original well record in our historical records database, it was revealed that TD is at 6854'. Additionally, further research shows the Tully Formation is much shallower in this area than 5664', and may be an error on the report.
