Well permit # 065-27044 (Sulgar Farms 4H # 50148) had also been approved for this site, but the permit expired on 9/24/2014 and the operator has reported that the well will not be drilled. The ESCGP-2 for this site expires in 2018.

2710077 Primary Facility 2018-03-26 Routine/Complete Inspection No Violations Noted Inspected location with OGI Walker. Well appears to be in production. No current issues at the time of the inspection except water level inside the tank containment needs to be addressed.

Since the previous inspection on 3/28/2017, the violation has not been corrected and there have been no changes at the site. This site is covered by ESCGP-2 # ESG13-065–0005. This ESCGP will expire on October 7, 2018. If the earth disturbance activities are not completed by this date, a new and complete NOI and E&S and restoration plan is required to be re-submitted and approved for continuation of earth disturbance activities.

The well integrity was in good condition, with no signs of any leaks or spills. No corrosion was noticed. The gauges on the well were reading 158 psi for the tubing and 350 psi for the production casing. There was a 50 bbl plastic tank set next to the well which looked to be empty. There were also 3 tanks within a lined, steel dike onsite. A line heater and separator were also being used. The well was properly labeled and the Emergency Response sign was posted at the site entrance.
