2792762 Primary Facility 2018-10-18 Routine/Partial Inspection No Violations Noted A notice of intent to plug for this well was to be sent in with work to begin ASAP. There was a potential gas migration in the area and this well fell within the 2500' buffer around the water well. I did not notice any gas coming from the well and the annulus was open since the valve was broken off. There was a road built to the well off of Baha Lane. The coordinates given were accurate.
2798160 Primary Facility 2018-10-30 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G was onsite attempting to fish casing. Both the 3" and 6" casings have parted in the well. The 6" was leaning to the side, so attempts were being made to straighten it. An overshot was run, but after several attempts, it was not working. A magnet was also run and metal chunks were brought back to surface. Plan was to get another tool to attempt to get the 6". They would attempt again tomorrow. It was noticed that the water level dropped in the hole and the bubbling appeared to have ceased.
2798385 Primary Facility 2018-11-05 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G was onsite working at pulling the 3" out of the well. The 6" that had broken off from last inspection was fished out of the hole with an overshot. A swedge was then run n to open up the casing that had crimped inward. So far, approx. 19' of 3" has been pulled, but has not made much progress.

While onsite, the 3" broke free and 2 joints were removed from the well. Things were still going well when I left the site.

2800515 Primary Facility 2018-11-07 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G was onsite cleaning out the well. The depth was 825'. All of the 3" was pulled out. A camera was run this morning. The top of the water was at 34'. Two videos were given showing that there were no bubbles in the well from gas.
2800523 Primary Facility 2018-11-08 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G was onsite cleaning out the well. The depth was 855'. A roll off tank was brought onsite. It was lined. While I was onsite, about 3' was made. The well was baled and getting mostly a gray clay with bits of rock.
2804960 Primary Facility 2018-11-14 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G was onsite cleaning out the well. The depth was 878'. About 6' of hole was made today so far. A very sticky clay was encountered that clogged the baler up.
2805466 Primary Facility 2018-11-20 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G was onsite running a swedge into the well to try and open up the 6", which had folded inward some. Some rubber from the packer was picked up so they plan to run a cherry picker in once they are done with the swedge. The depth was 1128'.
2809412 Primary Facility 2018-11-27 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G was just getting back into the well with a bit. They had been working an axe bit to chop up the packer rubber believed to be on bottom. Depth was at 1035'.

Note: Last inspection should have had depth at 1028', not 1128'.

2809414 Primary Facility 2018-11-28 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G was using a regular bit after using the axe bit again. Quite a bit of rubber was recovered yesterday. Depth was at 1035'
2811790 Primary Facility 2018-12-05 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G was using a barbed spear to stir things up. Hoping that they contact the slips. They believe that over 1/2 of the packer rubber has been retrieved. Depth was at 1060'.
2812125 Primary Facility 2018-12-07 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G was using a barbed spear to stir things up. They were able to retrieve the slips, but something else hard was preventing them from getting deeper. They ran a magnet while ai was onsite and got back small fragments of metal. Depth was at 1065'.
2818556 Primary Facility 2018-12-19 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G is down to 1075' and running a barbed spear. They are able to grab something and pull up about 5' before they lose it. No new hole has been made since Dec. 6th. While onsite, they switched off to an axe bit and drove it down to 1080' and became stuck. Will continue attempts to get deeper. They are going to try a double pronged spear tomorrow.
2818560 Primary Facility 2018-12-20 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G was doing maintenance on rig. The 2 pronged spear has been run, but could not break through what is in the well. The jars were removed from assembly to try and beat harder with he axe. They feel the hole is clean to 1075'.
2825072 Primary Facility 2019-01-03 Routine/Partial Inspection No Violations Noted At the time of inspection, a power swivel from Alco was brought onsite to use. They had made it to 1142' when I arrived onsite. When I was leaving, they were down to 1232', but ran out of water so hoses were being run to a nearby stream.
2825078 Primary Facility 2019-01-04 Routine/Partial Inspection No Violations Noted At the time of inspection, the power swivel was still being used. They pulled casing up overnight since cuttings were not circulating out of the well. They feared getting stuck. Something was giving them trouble after about 25' into drilling new hole and was causing a lot of torque. They had to pull out 30' and have only gained 15' back.
2825095 Primary Facility 2019-01-08 Routine/Partial Inspection No Violations Noted At the time of inspection, crew was setting up to go back downhole with power swivel. They had been having pump issues at the pit. They got it running and made it down to 1320'. At 9:30 AM, they were having issues again. When leaving location, the depth was at 1410'.
2825178 Primary Facility 2019-01-10 Routine/Partial Inspection No Violations Noted At the time of inspection, the well was given an attainable bottom of 1537'. They lost some footage overnight so they drilled back down and flushed the well. Cement trucks arrived around noon. Plants & Goodwin cementing 3 stages today. They did a 225 sxs followed by 225 more sxs and finished with 200 sxs Class A cement. This will set a plug from 1537'-787', which should be just below the 4" packer that had been set.

Tomorrow they plan to remove the 4" pipe and then attempt to retrieve the 6 5/8" casing. If pulling fails, the casing will be perforated and cement squeezed behind pipe. Top of cement in either case will be around 400'.

2828646 Primary Facility 2019-01-17 Routine/Partial Inspection No Violations Noted At the time of inspection, Plants & Goodwin arrived onsite to finish cementing. Just prior to cementing, the hydraulic box got a crack in it. Another truck had to be called in.

The 6" had been perforated yesterday with 32 holes. There were 4 shots every 30' or so starting above the set depth at 645'. The top of cement was currently at 800' and plans were to cement up to 380'. I left site at 10:30 AM and came back at 1 PM. 2nd cement truck arrived after lunch time. Cement completed just before 2 PM.

2832010 Primary Facility 2019-01-24 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G was onsite running a camera. This was to determine if there was any gas present after plugging. No bubble were recorded.

A load of pea gravel was brought onsite to fill the remainder of the well. A monument will then be set. There was some pipe remaining on the site, but all else has been moved out.
