2838069 Primary Facility 2019-02-06 Routine/Partial Inspection No Violations Noted This is a well that the operator plans to plug soon. The well was labeled and the E & S control plan was in a PVC tube at the entrance to the site. The location has been rocked.
2841365 Primary Facility 2019-02-14 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G has a rig set up over the well. A bridge was tagged at 720'. 7" is at 748'. Well has 2 man-made bridges set by NFG. The top one is from 780'-800', consisting of stone and 2 sxs cement.
2846027 Primary Facility 2019-02-25 Routine/Partial Inspection No Violations Noted At the time of inspection, G & G has cleaned out well to 2785'. TD of well is at 3054', however, there is cemented casing/tubing that was cut off and left in the well. The top of the 2" tubing stub is at 2785'.

Since this would be very difficult to get through, attainable bottom was granted. An alternate method was requested. Plan had been to pump 30 sxs cement on bottom, which should be around 150' of a plug, but something was drinking the water/gel prior to cementing. So the new plan is to use 80 sxs cement on bottom and tag it tomorrow.

2847446 Primary Facility 2019-02-28 Routine/Partial Inspection No Violations Noted At the time of inspection, the 1st cement plug had been set and tagged at 2445'. The Speechley formation was plugged using 100 sxs, which was tagged at 2022'. The 1 1/2" was pulled out. The 4 1/2" was being pulled out when the packer became tight. Rotating counter-clockwise to work the packer free, the pipe unscrewed and fell down the hole. The top of the casing was found at 680' and packer is at 779'.

The plan had been to perforate the 7" from 648' to 728'. With the 4 1/2" there, plan is to try to screw back on to it, then cut and pull. If that fails, Surefire will use a perf gun that can shoot through both the 4 1/2" and 7" casings. Cement would then be squeezed through.

2852778 Primary Facility 2019-03-13 Routine/Partial Inspection No Violations Noted At the time of inspection, well had been filled with pea gravel from 557' to surface. The permanent marker as set as an above ground steel pipe. The API number, well name and plug date are stamped on to a button, which was set at the ground level. The permit posting sign is also at the well.
